Objective:1To investigate the expression levels of HO-1and lipid metabolismindicators in gestational diabetes mellitus and to explore the correlation ofboth.2To investigate the correlation between HO-1and the severity ofgestational diabetes mellitus. Expect to provide new ideas for clinicaldiagnosis and treatment.Methods:60women with diabetes during pregnancy enrolled as studygroup, which was divided into diabetes mellitus group (DM) and gestationaldiabetes mellitus group (GDM).35pairs of normal ones served as controls.Height, pre-pregnancy weight, admission weight and blood pressure of allpregnant women were recorded. Calculate all pregnant women’s body massindex (BMI). Maternal peripheral blood specimens were collected for HO-1measurement by ELISA, and lipid metabolism indicators were assessed at thecenter of blood test in our hospital. Placental tissues are collected forobserving HO-1expression in placenta by immunohistochemical. All the datawere analyzed by SPSS13.0software.Results:1Serum HO-1values in DM group, GDM group and control group were(83.57±16.50)ng/ml,(78.12±18.35)ng/ml and (41.50±14.76)ng/mlrespectively, DM group and GDM group were significantly higher thancontrol group (P<0.05, P<0.05). Compared with DM group, the serum HO-1value in GDM group showed no significant difference (P>0.05).2Among the index of lipid metabolism: Serum TG values in DM group,GDM group and control group were (4.73±1.78)mmol/L,(4.55±2.30)mmol/L and (2.21±0.85)mmol/L respectively, DM group and GDM group were significantly higher than control group(P<0.05, P<0.05),Compared with DM group, the serum TG value in GDM group showed nosignificant difference (P>0.05); Serum HDL values in DM group, GDM groupand control group were (1.68±0.80)mmol/L,(1.70±0.55)mmol/L and (2.19±0.70)mmol/L respectively, DM group and GDM group were significantlylower than control group(P<0.05, P<0.05), Compared with DM group, theserum HDL value in GDM group showed no significant difference (P>0.05);Serum LDL values in DM group, GDM group and control group were (4.03±1.41)mmol/L,(3.77±1.21)mmol/L and (3.05±0.87)mmol/L respectively, DMgroup and GDM group were significantly higher than control group(P<0.05,P<0.05), Compared with DM group, the serum LDL value in GDM groupshowed no significant difference (P>0.05). The rest of the various indicatorshave no significant differences between groups.3Serum FPG values in DM group, GDM group and control group were(6.33±1.65)mmol/L,(5.60±0.83)mmol/L and (4.33±0.55)mmol/Lrespectively. There was significant difference (all P<0.05); HbA1c values inDM group, GDM group and control group were (7.55±0.91)%,(5.86±0.33)%and (5.57±0.22)%respectively. There was significant difference (allP<0.05).4Positive correlation was founded between serum HO-1values and thelevels of FPG, HbA1c, TG, LDL(r=0.562,P=0.001; r=0.436,P=0.016;r=0.727,P=0.000; r=0.615,P=0.000), while the negative correlation wasfounded between HO-1values and the levels of HDL(r=-0.526,P=0.003)inDM group. Positive correlation was founded between serum HO-1values andthe levels of FPG, HbA1c, TG, LDL(r=0.478,P=0.007; r=0.408,P=0.025;r=0.699,P=0.000; r=0.534,P=0.002), while the negative correlation wasfounded between HO-1values and the levels of HDL(r=-0.705,P=0.000)inGDM group. Serum HO-1concentrations have no relation with BMI, CHOL,APOA, APOB (all P>0.05).5HE staining: In the control group, structure and morphology ofplacental tissue reveal normal and villus mature well. Trophocyte in villus surface have no stratification and be arranged regularly. In GDM and DMgroup, the placental villous is disorder and mature bad. Villi vessel wall isthickening. Vascular in villous stroma overfill.6HO-1were expressed in placental of both normal pregnancy anddiabetic pregnancy. HO-1expression was in cytoplasm. They were graduallyrising in the control, GDM and DM group.Conclusions:1Serum value of HO-1rise significantly in gestational diabetes mellitusand it related to blood sugar, which may play an important role of thedevelopment of gestational diabetes mellitus.2TG and LDL of gestational diabetes mellitus were higher than normalpregnant women, while HDL is below normal pregnant women.3Serum HO-1concentrations correlated positively with TG and LDL,correlated negative with HDL, so HO-1may be mediated the regulation oflipid metabolism.4The increase of placenta HO-1expression level in gestational diabetesmellitus may be the protection of the body and the fetus.
