Objective: The retrospective study is concerned about gender, time oflife, number of lesion, the degree of differentiation, depth of invasion, tumorlocation, intravascular cancer embolus, diameter of tumor, subtypes andpresence of an ulcer of early gastric carcinoma. To analyze the association thatthe risk factors of lymph node metastasis with early gastric cancer(EGC)andpredict lymph node metastasis with early gastric cancer(EGC), to providereferences for method of the reasonable treaty and appraisal prognosis.Methods: One hundred and four patients with EGC removed surgicallyfrom Mar2003to Mar2013were analyzed retrospectively, collect thematerials of clinic, operation, pathology. Analyse the rule of the lymph nodemetastasis. the clinicopathologic features and lymph node metastasis wereanalyzed by univariate analysis and multivariate analysis.Results: This is a retrospective analysis of the material of104cases ofearly gastric carcinoma treated. Analyse104cases,74were female sex and30were male sex. The ratio of men and women about2.47:1, Age from36to75,the median patient age is58years, There are92cases with single lesion,12cases multiple lesion, All samples are divided into the high or middledifferentiation group(46) and the low or no differentiation group(58),According to depth of invasion, there are86intramucosal carcinoma cases andothers are submucosal cancer, There was12patients whose tumor positionwas upper stomach,26patients whose tumor position is stomach central,66patients whose tumor position is lower, There are10patients withintravascular cancer embolus, There were18patients are protrude type,36patients are superficial type,50patients are umbilicate type stomach cases,there are54patients with an ulcer in the tumor, There are82patients whosediameter is≤2cm, and there are22patients whose diameter>2cm. Lymph node metastasis: There were12patients who has lymph node metastasis.Univariate analysis revealed correlations between lymph node metastasis andvarious factors ranging from, the degree of differentiation(P=0.041), depth ofinvasion(P=0.000), Intravascular cancer embolus(P=0.003), presence of anulcer(P=0.021) and diameter of tumor(P=0.001). There are no correlationsbetween lymph node metastasis and various factors ranging from gender, timeof life, number of lesion, subtypes, tumor location of early gastric carcinoma.However, in multivariate analysis, the degree of differentiation(P=0.010),invasion to the submucosal(P=0.000), intravascular cancer embolus(P <0.004),presence of an ulcer (P=0.006)and diameter of tumor(P=0.001), were found tobe independent risk factors.Conclusions: Compared to advanced gastric cancer, early gastriccancer have a better prognosis. Although surgical treatment But there are stilla recurrence or metastasis, Lead to a vicious outcome.It is reported thatLymph node metastasis are independent risk factors of early gastric cancerprognosis and determine the key factors of surgical procedure.In case of thePresence of an ulcer, invasion into the submucosal and vascular invasion, thetumor diameter greater than20mm, Poorly differentiated in patients withearly gastric cancer, lymph node dissection should be considered. To analysethe early gastric cancer risk factors, for lymph node metastasis, predict lymphnode metastasis risk, and provide evidence for reasonable treatment scheme,improve patient quality of life.
