| Part One:To explore the clinical curative effect between the pattern of internal accumulation of zhuodu of CAG and HuazhuojieduFormula based on correspondences between prescription and syndromesObjectives: To research correspondences between prescription andsyndromes on the pattern of internal accumulation of zhuodu of chronicatrophic gastritis(CAG),and identify inherent connections and rules with thepattern of internal accumulation of zhuodu of CAG and HuazhuojieduFormula whose therapeutic principle is huazhuojiedu method. Thus providingguidance for clinical practice and improving the clinical effects in CAGeffectively.Methods:164patients who suffered CAG were divided into three groups,prescriptions corresponding to syndromes group of65patients, and syndromesnot corresponding to prescriptions group of48patients, which all take orallyhuazhuojiedu formula; prescriptions not corresponding to syndromes group of51patients, which take orally qingrelishi formula. To observe continuouslytwo treatments which have three months for a course of treatment, andcompare before and after treatment of the change of Clinical symptomsã€gastroscope images and gastric mucosa pathological histology.Results:(1)Changes of the shows of TCM symptoms before and aftertreatment in three groups:the total efficiency of prescriptions corresponding tosyndromes group was90.78%, the total efficiency of prescriptions notcorresponding to syndromes group was70.59%, the total efficiency ofsyndromes not corresponding to prescriptions group70.84%. In the total effective rate of improving the TCM syndromes with three groups beforeand after treatment,the prescriptions corresponding to syndromes group wassignificantly higher than prescriptions not corresponding to syndromes group,syndromes not corresponding to prescriptions group(P<0.05),and there wasno remarkable significance (P>0.05) of the comparison betweenprescriptions not corresponding to syndromes group and syndromes notcorresponding to prescriptions group. Thus prescriptions corresponding tosyndromes group was superior to the other two groups.(2)There were changesbefore and after treatment of three groups in endoscopic gastric mucosalcondition: in prescriptions corresponding to syndromes group, the totaleffective rate was93.31%, the total efficiency of prescriptions notcorresponding to syndromes group was70.59%, the total efficiency ofsyndromes not corresponding to prescriptions group72.92%. In the totaleffective rate of improving the endoscopic gastric mucosal with three groupsbefore and after treatment, there was significant difference(P<0.05) betweenthe prescriptions corresponding to syndromes group and the other two groups,and there was no remarkable statistical significance (P>0.05) betweenprescriptions not corresponding to syndromes group and syndromes notcorresponding to prescriptions group.Thus the prescriptions corresponding tosyndromes group was superior to the other two groups.(3)There were changesbefore and after treatment of three groups in inherent glandular atrophy of thegastric mucosa, gastric intestinal metaplasia, gastric mucosal atypicalhyperplasia:in prescriptions corresponding to syndromes group, the totaleffective rate was84.62%, the total efficiency of prescriptions notcorresponding to syndromes group was62.75%, the total efficiency ofsyndromes not corresponding to prescriptions group60.42%.In improvinggastric intestinal metaplasia, the total efficiency of prescriptions correspondingto syndromes group was86.15%, the total efficiency of prescriptions notcorresponding to syndromes group was62.75%, the total efficiency ofsyndromes not corresponding to prescriptions group62.50%. In improvinggastric mucosal atypical hyperplasia, the total efficiency of prescriptions corresponding to syndromes group was83.08%, the total efficiency ofprescriptions not corresponding to syndromes group was56.86%, the totalefficiency of syndromes not corresponding to prescriptions group58.33%. Inthe total efficiency, the difference was statistically significant (P<0.05)in theprescriptions corresponding to syndromes group and the other two groups, andthere was no statistical significance (P>0.05) of the comparison betweenprescriptions not corresponding to syndromes group and syndromes notcorresponding to prescriptions group.Thus the prescriptions corresponding tosyndromes group was superior to the other two groups.Conclusions:The present research found that Huazhuojiedu Formulacan obviously improve clinical symptoms, endoscopic gastric mucosalcondition of CAG patients with the pattern of internal accumulation ofzhuodu,also can significantly postpone and reverse gastric mucosal intestinalmetaplasia and atypical hyperplasia.But they had poor effect whenHuazhuojiedu Formula treated others’ syndromes, and other prescriptionstreated the pattern of internal accumulation of zhuodu. Therefore the researchconfirm correspondences between prescription and syndromes with the patternof internal accumulation of zhuodu of CAG and Huazhuojiedu Formula whosetherapeutic principle is huazhuojiedu method from the perspective of clinicalpractice. Applied Huazhuojiedu Formula to the pattern of internalaccumulation of zhuodu of CAG,the therapeutic effect is the best.Part Two:To research the rule in treating the pattern of internal accumulationof zhuodu of chronic atrophic gastritis(CAG)Objectives:Using data mining and statistical analysis method, to studymedication rules of the use of Huazhuojiedu Method to treat CAG bysummarizing and analyzing clinical cases retrospectively. And from this angleto explore stable relationship of correspondences between prescription andsyndromes in the two sides, making the relationship more clear, objective andaccurate, so that the result can guide clinical practice preferably.Methods: Statistical analysis was finished among the200cases of CAG, which were outpatients of the Department of Gastrointestinal in HebeiProvincial Hospital of TCM, about their gender, age, clinical symptomsprescriptions of TCM. Taking golden section method, to apply data miningmethods including frequency statistics method and association rules analysis,to analyze association rules between drugs and syndromes, explore thecorrespondences between prescription and syndromes.Results: Through data processing, there were153kinds of traditionalChinese Medicine, based on200cases of traditional Chinese medicineprescription. They were ascribed to18kinds by their category,includingrefrigeration drugsã€regulating Qi drugs and tonic drugs were used the mostfrequently. According to golden section method,15kinds of traditional Chinesemedicine, whose frequency of use over61.8%,were therapeutical core drugsof CAG.10kinds of clinical symptoms appearing mostly, were cardinalsymptoms of CAG. Discovery by study: the association between symptomsand pulses, which explained the high degree of correspondences betweenprescription and syndromes in the pattern of internal accumulation of zhuoduof CAG and Huazhuojiedu Formula.Conclusions:By establishing the basic database, applying data miningmethods including frequency statistics method and association analysis toanalyze gender, age, clinical symptoms, drugs frequency and classification,therelation of prescription and syndromes. It revealed objectivelycorrespondences between prescription and syndromes in the pattern of internalaccumulation of zhuodu of CAG and Huazhuojiedu Formula. To embody thatthe key pathogenesis of CAG is internal accumulation of zhuodu, and thecorresponding treatment is Huazhuojiedu Method. |
