| BackgroundMajor depression is the emphasis and difficulty in clinical psychiatric, it’s difficult to have efficacy when use anti-depression drugs on some depression patients who have severe depression symptom or strong suicidal ideation and behavior. It’s difficult to have efficacy and hardly to control the disease when use anti-depression drugs on some depression patients who have severe depression symptom or strong suicidal ideation and behavior. Modified electroconvulsive therapy has widely used in clinical psychiatric, especially when combined with drugs would have a better efficacy, but the cognitive function effect of this therapy is still vague. P300and EEM have widely used in clinical psychiatric diagnosis, when compare to the neuropsychological tests, they are hardly affected by the external environment, so P300and EEM are the better cognitive function evaluation. In this study, by analyzing the diversification of P300and EEM before and after the treatment, we can explain the impact of treatment on cognitive function better.ObjectiveTo analyze the diversification of neurophysiological indicators after using drug combined MECT and drug treatment on major depression disorders respectively, to investigate the effect of drug combination MECT treatment on cognitive function.Methods1. Select the eligible subjects who were diagnosed major depression whatever outpatient treatment or hospitalization from May2011to November2011in Nanyang psychiatric hospital. All patients were randomly divided into combined group(n=50) and drug group(n=50),the patients in drug group were treated by anti-depression drug for6weeks, and the patients in combined group were additionally given the MECT1course besides anti-depression drug treatment. The modified electroconversive therapy were used treatment instrument made by American Eagle Dorset Company.2. We detected all patients depression depressive symptoms by using the Hamilton Depression Rating Scale(HAMD) before treatment and the day after treatment over, and recorded the change of HAMD total scores and reducing rates;3. We detected all patients’ P300index by using the event-related potential instrument(Made by Medical Devices Co., Ltd., Guangzhou Runjie) before treatment and the day after treatment over, and recorded the change of P300-Latency and P300-amplitude.4. We detected all patients’ EEM index by using the exploratory eye movement experiments instrument (Made by Medical Biotechnology Co., Ltd. on the Hai Dikang)before treatment and the day after treatment over, and recorded the change of NEF and RSS.5. We compare the change of HAMD,P300and EEM between the two group before and after treatment, the intragroup and intergroup data were compared with the t-test, and we using the bivariate correlation analysis for the HAMD scores and P300,EEM of combined group.Results1. There were no significantly differences of the visual-P300/EEM/HAMD between the two groups(P>0.05).2. When compared with pre-treatment, the HAMD scores of drug group patients were significantly lower (P<0.01), and the P300-latency were significantly shorten (P<0.01) and the P300-amplitude were significantly decreased (P<0.01).While there were no significantly differences on the RSS and NEF of EEM(P>0.05).3. When compared with pre-treatment, the HAMD scores of combined group patients were significantly lower (P<0.01), and the P300-latency were significantly shorten (P<0.01) and the P300-amplitude were significantly decreased(P<0.01).Besides, the scores of RSS and NEF were also significantly increased(P<0.01).4. When compared with drug treatment group, the HAMD scores and the reduction rates of combined group were significantly decreased (P<0.01), and the P300-latency and the index of EEM were also significantly changed (P<0.01).5. There were highest relevance of HAMD score and P300-latency in combined group, the results had significant differences(P<0.01). The other three indicators were weakly correlated, does not reach the level of significant difference(P<0.05).Conclusions1. Simple drug treatment have efficacy on severe depression patients, and it can improve the index of P300while have no effect on EEM.2. MECT combined drug treatment have efficacy on severe depression patients, and this treatment could improve the P300and EEM index more significantly.3. MECT combined drug treatment have better efficacy than simple drug treatment, and it can improve neurophysiological index more significantly, so we consider this treatment may not cause cognition damage.4. The treatment of drug combined MECT maybe improve the major depression patients’ cognitive function by easing the depression symptom. |
