The Clinical Analysis Of Childhood Vitiligo Prognosis Assessment By Integrative Medicine Treatment

Thesis Purposes:1. Establish the clinical characteristics and prognosticassessment model for children vitiligo. And utilize the model to estimate the skin lesionprognosis for children vitiligo patients with given survey information.2. Provide thedoctors with references for clinical treatment choice and optimize the therapeuticprograms.3. Use the preliminary research results to build the “Children VitiligoPrognosis Assessment Software” and spread the children vitiligo prognosis assessmentsoftware through internet and provide the basis to reclaim more clinical data.Methods:1. Research for every skin lesions and establish a complete medicalhistory through literature search. A complete medical history would be a combination ofpatient’s and doctor’s records.2. Collect volunteer children vitiligo patients’(2to14years old) clinical characteristics information, which includes gender, age, births,breastfeeding, health situation, exercise situation, picky eater situation, course of disease,vitiligo family history, concomitant diseases, halo nevus, post-traumatic excitationleukoplakia, skin lesion distribution, skin lesion position, skin lesion shape,pigmentation around skin lesions, white hair around skin lesions, area of the skin lesion,skin lesion types, skin lesion periods, disease history prognostic characteristics, Chinesetraditional medicine syndromes, periods of the treatment, PUVA exposure time,treatment specification and curative effect.3. The therapeutic program is combined withtraditional Chinese and western medicine, which includes Chinese traditional medicineand Chinese medicine, oral microelements, vitamins combined with PUVAphotochemotherapy.4. Process the clinical data with statistical analysis using theSPSS20.0. And build the model with multinomial Logistic regression analysis.Results:1. In this research,60patients were admitted into the group, no one drop out, and150skin lesions were observed. After integrative medicine therapy (IncludingChinese traditional medicine and Chinese medicine, oral microelements, vitaminscombined with PUVA photochemotherapy.)24%were cured,21%were markedlyeffective,20%were effective and34.7%were invalid, the overall effective rate was65%.2. In the univariate analysis、age、exercise、Chinese traditional medicine syndromes、skin lesion distribution、skin lesion type and periods of the treatment factors haveheterosceasticity and they were excluded. The remaining18factors were brought intothe multinomial Logistic regression analysis.3. PUVA exposure time, skin lesionposition, white hair around skin lesions, area of the skin lesion and treatmentspecification are considered as impact factor and incorporated into the regression model.4. The model coefficient values are acquired from the “parameter estimation” and themodel is then set up based on these values.5. Based on the above information, thechildren vitiligo prognosis assessment software can be presented.Conclusions:1. This research shows that PUVA exposure time, skin lesionposition, white hair around skin lesions, area of the skin lesion and treatmentspecification are significant to predict children vitiligo.2. The efficacy of integrativemedicine therapy (Including Chinese traditional medicine and Chinese medicine, oralmicroelements, vitamins combined with PUVA photochemotherapy.) was significantly,the overall effective rate was65%.3. Multinomial Logistic regression analysis is usefulto the analysis of children vitiligo clinical characteristics and prognostic assessment.Our research has established the children vitiligo clinical characteristics and approacheda children vitiligo prognosis assessment model preliminarily. We are able to makeprognostic assessment for children vitiligo patients with given information, and presentthe assessment results with percentages.4. The children vitiligo prognosis assessmentsoftware can display the results clearly.
