| Objective: To observe the effect of using posterior injured-vertebra pedicle screw-rodsystem fixation to treat the thoracolumbar fractures in young,and provide certain referencefor the clinical treatment.Methods: A retrospective analysis about44cases of20-55years old thoracolumbarcompression and burst fractures,were divided into the observation group and the controlgroup,22cases in each group.The observation group was treated with through-injured-vertebra pedicle screw-rod system fixation,and the control group for the across-injured-vertebra pedicle screw fixation.All cases had no symptoms of spinal cordinjury,and the damage location was between T11and L2.Preoperative X-ray,CT and MRIexamination,and postoperative follow-up were obligatory.Follow-up time was at least1years,and regularly checked using the image.Recorded and compared the operationtime,bleeding and the VAS score,vertebral compression rate,kyphotic Cobb angle ofpreoperative,postoperative day2and the last follow-up,and the vertebral height andkyphosis Cobb angle loss at the last follow-up.Results:Two cases of preoperative difference comparison of general data showed nostatistical significance(P>0.05).During follow-up,the control group had2cases of internalfixation loosening,1case of pedicle screw fracture,and the observation group had no caseof the above.The pain was significantly relieved in two cases after operation,andcomparing the difference between the two groups was not statistically significant(P>0.05).The observation group's operation time and bleeding volume during operationwere higher than those in the control group, the difference was the statistical significance (P<0.01).In the observation group after2days,the vertebral compression rate and Cobbangle were significantly smaller than that in the control group,the difference wasstatistically significant(P<0.05).At the last follow-up,in the observation group loss ofvertebral height and angle were significantly less than the control group,there wassignificant difference(P<0.01)Conclusion:By through-injured-vertebra pedicle screw fixation and traditionalacross-injured-vertebral pedicle screw fixation can promptly, effectively relieve the pain ofthe patients,and the short-term and long-term effect are satisfied.But withthrough-injured-vertebra pedicle screw fixation distraction reduction effect is better.Itmore can significantly restore vertebral body height, effectively improve the local kyphosisCobb angle,keep the vertebral body height and the normal physiological curvature forlong-term after reset, reduce the loss of vertebral height and Cobb angle in future,preventthe loosening and fracture of internal fixation.Enhance the postoperative stability alsohelps to exercise activity and function in patients with early.In spite of this,the operationtime will be extended and hemorrhage volume will also increase which may improve therisk of operation.So we should strictly control the indications. |
