Objective: To investigate the cause, diagnostic methods and prognosisof fever of unknown origin in children.Methods: Clinical data and prognosis of104patients admittedbetween January1,2005and December31,2011with FUO were analyzedretrospectively. They were divided into three groups according to age:Younger than3years, Between3and7years, older than7years.Results:1. Generals: Clinical data of74boys(71.15%) and30girls(28.85%)were analyzed, the average age was4.86±4.24years.47cases wereyounger than3years old,24cases were between3years to7years,33cases were older than7years.2. Diagnostic methods: History, physical examination and therapeuticefficacy contributed to the diagnosis in38cases (36.54%). Serology oretiology, imageology, biopsy,bone marrow examination and endoscopyaccount for diagnosis in24cases (23.08%),16cases (15.38%),6cases(5.77%),4cases (3.85%),1cases (0.96%) respectively.3. Etiology: The definite diagnosis was finally achieved in89cases, the definite diagnostic rate being85.58%.(1) In Younger than3years group,23cases of47cases (48.93%) had infectious diseases.5cases (10.64%) hadconnective tissue diseases.1case (2.13%) had tumors and malignantdiseases.(2) In between3and7years group,11cases of24cases (45.83%)had infectious diseases.4cases (16.67%) had connective tissue diseases.5cases(20.83%) had tumors and malignant diseases.(3) In older than7yearsgroup,6cases of33cases(18.18%) had infectious diseases.19cases(57.58%) had connective tissue diseases.2cases(6.06%) had tumors andmalignant diseases.4. Prognosis: Follow up data were obtained in61cases of the104cases (58.65%).27cases(44.26%) were cured of the61cases.26casesbenefited from medical therapy.2cases had serious complication.6casesdied.Conclusions: Infectious diseases was the major causes of FUO ininfants. Infectious diseases, connective tissue diseases,tumor and malignantdiseases were the major three causes of FUO in pre-school age and schoolage children. The prognosis of infectious disease and connective tissuediseases was better than tumor and malignant diseases in children with FUO.If early diagnosis can be made, tumor and malignant diseases may achievegood prognosis with standard treatment. |
