ObjectiveTo investigate and analyze the significance of treatment in severesudden sensorineural hearing loss patient, which without standard treatmentor poor treatment effect present for at least one week.MethodsWe retrospective59patients with severe sudden sensorineural hearingloss (SSNHL) who pure tone average (PTA) threshold was more than70dBand present for at least one week and they were admitted to the department ofOtorhinolarngology-Head and Neck Surgery, The First Affiliated Hospitalof Chongqing Medical University from January2011to January2013.Thepatients were devided into severe, profound and total SSNHL by the hearingthreshold. All of the patient were tearted with glucocorticoids,thrombolytic, and used hyperbaric oxygen therapy or intratympanicmethylprednisolone as remedial treatment. Results1.The improvement in PTA threshold among the severe, profound andtotal SSNHL groups was separately19.7±14.6dB,20.6±18.6dB and13.5±4.4dB, the diffrence of which has statistical significance(P<0.05).2.The short-term effictive treatment rate of severe, profound and totalSSNHL groups was47.8%、48.3%、50%and there was no statisticsignificance difference among the groups(2=0.0113, P>0.05).3.After therapy, the percentage rate of of severe, profound and totalSSNHL groups with PTA≤50dB was39.1%、13.8%、0, the difference ofwhich has statistical significance. The severe SSNHL was the highest amongthe three.4. The improvement in PTA threshold of the severe-above groupslasting for7to13days was19.7±14.6dB, the diffrence of which hasstatistical significance(P<0.05). The improvement in PTA thresholdbetween the severe-above groups lasting for7to13days,14to29days ormore than30days were separately20.6±18.6dB and13.5±4.4dB, thediffrence of which has no statistical significance(P>0.05).5.The efficieive rate of the severe-above groups lasting for7to13days,14to29days or more than30days were separately60.5%,28.6%,25%, thediffrence of which has statistical significance(2=6.1959, P<0.05).6.After therapy, the percentage rate of the severe-above groups withPTA threshold less than50dB lasting for7to13days,14to29days, morethan30days was26.3%、14.3%、12.5%, the diffrence of which has no statistical significance(2=1.3294,P>0.05).ConclusionSevere SSNHL need treatment as soon as possible. It does have effectfor patients in severe sudden sensorineural hearing loss under poor ornon-standard treatment lasting for more than one week. However, the resultmay not good for those who are in severe SSNHL for more than two weeks.
