| PART ONE THE EVALUTION OF THORACIC DEFORMITY INPECTUS EXCAVATUM OF CHILDREN WITH64-SLICECOMPUTED TOMOGRAPHYObjective:Through the retrospective research of thoracic morphology and thoracicscoliosis in pectus excavatum of different age groups,we investigate therelationship between thoracic asymmetric, scoliosis and age.and investigatethe diagnostic value of64-slice spiral CT at the same time.Methods:64-slice CT and three-dimensional reconstraction features of pectusexcavatum proved clinically162cases between2009and2011wereretrospectively reviewed,According to Park classification, divided pectusexcavatum into symmetric and asymmetric.Observe sternum, rib cartilage and scoliosis, measure the Haller index,Use age as the classificationstandard classified the cases to three grades, analyse differences betweenthe three grades on Haller index, thoracic asymmetric, scoliosis, andinvestigate the relationship between thoracic asymmetric, scoliosis and age.Results:162cases were classified three groups:0~3age group:81cases,3~6agegroup:35cases,6~age group:46cases,the Haller index of the three groupsrespectively was3.76±0.98,4.14±0.86,3.91±1.17,the three groups were notsignificant different(P>0.05).77cases out of the162cases wereasymmetric pectus excavatum, the three groups respectively was27cases(33.3%)ã€21cases(60.00%)ã€29cases(63.0%),there are significantdifferences between the three groups(P<0.05).33cases out of the77caseswith asymmetric pectus excavatum appear scoliosis. The three groupsrespectively was11case(13.6%),10case(28.6%),14case(30.4%),there are significant differences between the three groups(P<0.05).Conclusion:The64-slice CT is a better tool for revealing horacic morphology andthoracic scoliosis of pectus excavatum. As the age increase, thoracicasymmetry and scoliosis are increase. PART TWO THE EVALUATINO OF THE CARDIAC ANDSAMALL AIRWAY CHANGS IN PECTUS EXCAVATUM OFCHILDRENWITH64-SLICE COMPUTED TOMOGRAPHYObjective:Throngh the retrospective research of cardiac rotation and small airwaychanges in pectus excavatum,we investigate the effect of the degree ofsternal depression on them and the value of the64-slice CT.Methods:64-slice CT and three-dimensional reconstraction features of pectusexcavatum proved clinically183cases between2009and2011wereretrospectively reviewed. Measure the Haller index, cardiac rotation angleon axial CT images of the chest, research the small airway changes on MPRlung-window.Use Haller index as standard classified the cases to threegrades, analyse differences between the three grades on cardiac angle, smallairway changes, and investigate the effect of the degree of sternaldepression on them.Results:183cases were classified three grades: mid-grade:54cases(2.86±0.31),middle-grade:22cases (3.36±0.10), high-grade:107case(4.61±1.27),the three gardes were significant differrents (P<0.05). Thecardiac rotation angle of the three grades were5.52o± 8.76o,57.38o±5.03o,65.93o±7.67,there were significant differrents amongthem(P<0.05).60cases have small airway changes, the mobidity of the threegrades were22.22%(12/54),22.73%(5/22),40.19%(43/107),there weresignificant differrents amony the three grades(P<0.05).53cases among themhad lobus pulmonis sinister small airway changes.Conclusion:The64-slice CT is a better tool for revealing chest deformity, cardiacrotation and small airway changes of pectus excavatum. As the degree ofchest deformity increase, the cardiac rotation angle, morbidities of smallairway changes are increases, the three exist positve correclation. |
