Psychological Status, Visual And Health Related Quality-of-Life Of Chinese Uveitis Patients

Backgroud and object:Uveitis is a chronic, relapsing ophthalmological disease, is one of the common blind eye diseases, which give the patients a great decrease in physical health and mental health and the quality of life. Many of uveitis patients need the long-term hormone therapy. More seriously, some even need to take immunosuppressive drugs to alleviate the condition. On the case of not be completely cured, the purpose of seeking to heal completely shifted from to pursue a relatively healthy goal, so the patient’s subjective experience that perceived a health status should be pay more attention. Uveitis as an ophthalmic disease, which mainly makes adverse effect to the patient quality of life through weak-eyed vision, therefore, we should study the patient’s quality of life from both the aspects of the general health and the eyesight condition. In2001,Schiffman et al studied of health-related quality of life and vision-related quality of life on the76uveities patients, they found that the Uveitis not only have a deeply adverse impact to the patient’s quality of life, but is one of the eye diseases which have greater influence to the patient. However, Schiffman and others only to do the small number of research samples, and the research samples is Westerners, so it is meaningful to study a large sample size of uveities patients direct the Chinese people. We investigated the patients of the Uveitis in psychological and physiological effects relationships of the health-related quality of life and the vision-related quality of life. We hope that this research will draw the ophthalmologist’s attention, besides, provide a reference for clinical treatment.Methods:1105Uveitis patients, Including the185BD (Behcet’s disease) patients,214VKH (Vogt-Koyanagi-Harada syndrome) patients,706non-infectious uveitis (non-infectious uveitis) patients, given HADS (Hospital Anxiety and Depression Scale) questionnaire before eye examinations, NEI VFQ-25(25-item National Eye Institute Visual Function Questionnaire), SF-36(Short-Form Health Survey). In addition, there are included114healthy people in the study as control group. The content of survey includes the patient’s age, sex, educational level, occupation, average monthly income, how long the duration of the illness and the patient’s vision. After obtains the survey data, According to the above questionnaire standard algorithm calculates the scores of each dimension, we comparison between each group of uveitis patients and comparison between groups of the uveitis and healthy control groups by single factor analysis of variance. Finally, use multiple regression analysis to find the relationship of the above factors and the pathogenesis of the uveitis diseases. Results:(1) Compared with health groups:anxiety and depression scores in uveitis patients were significantly higher than health (P<0.01), all dimensions of the health-related quality of life and all dimensions of the vision-related quality of life of uveitis is apparently lower than normal individuals(P<0.01);(2) Compared the score of the anxiety depression with patients groups:the groups of the Behcet’s patients was significantly higher than noninfectious groups of uveitis (P<0.05), but it is no difference between the Koyanagi-Harada syndrome disease and;(3) Behcet’s disease, compared the score of the vision-related quality of life with each groups:with vision-related quality of life, the three dimensions of the general health (GH), general vision (GV), role limitation (RL) is Significantly lower than Koyanagi-Harada syndrome patients and non-infectious uveitis patient group (P<0.05), and other dimensions is not Conspicuous different with Koyanagi-Harada syndrome patients, but was still Conspicuous lower than that of non-infectious uveitis group (P<0.05), only the dimension of the eyes and pain (OP) were not significantly different with the other two groups;(4) Compared the score of the health-related quality of life with each groups:Physiological function (PF), role limitations due to physical function (RP), general health (GH), social function (SF) were significantly lower than Koyanagi-Harada syndrome patient group and non-infectious uveitis group (P<0.05), bodily pain (BP) and vitality (VT) is no significant difference among the three groups, but the limited role due to mental health (RE) in this dimension has a significant differences in comparison of the Behcet’s disease patients and the Koyanagi-Harada syndrome (P<0.05);(5) Regression Analysis: vision-related quality of life and good eyesight, bad eyesight, anxiety, depression, average monthly income per person, age, work-related, can explain about60%variation.Conclusions:(1) The Uveitis patients has significant anxiety and depression, and the Behcet’s disease patients with depression most seriously.(2) Quality of life in uveitis patients, the vision-related quality of life and health-related quality of life was reduced, and the decline in Behcet’s patients more obvious.(3) the Uveitis patients quality of life have related to many factors, such as vision, mental situation,average monthly income, age and whether worked or not;(4)in clinical, doctors should be pay more attention to the quality of life and mental status of Uveitis.
