Part One:Risk Factors for Verruca Vulgaris:a Case-Control StudyObjectivesTo explore the major risk factors of verruca vulgaris, evaluate the relative importance of each risk factor and provide scientific guidelines for prevention and diagnosis of verruca vulgaris.MethodsCase-control study was designed, unification questionnaire was used.The data was entered by Epidata3.0software, x2test and unconditional logistic regression analysis were used for univariate and multivariate analysis of verruca vulgaris risk factors.SPSS13.0software was used for all the statistical analysis.ResultsUnivariate analysis showed that verruca vulgaris was associated with age under30years old,student,low-income,lower level of education,staying at home,smoking habits,lack of or excessive exercise,sleeping late,lack of sleep,living with verruca vulgaris patients,sharing slippers with other people,stress,tinea pedis,hyperhidrosis, local skin damage(P<0.01) and engaged in manual labor,living in humid place,tinea manus(P<0.05).Multivariate unconditional logistic regression analysis showed that lack of or excessive exercise,engaged in manual labor, local skin damage, living with verruca vulgaris patients,smoking habits,stress, hyperhidrosis,sleeping late,staying at home,age under30years old,lower level of education(P<0.01)and student,sharing slippers with other people(P<0.05) were significant risk factors of verruca vulgaris.ConclusionEnvironment,living habits and personal factors were the most important factors associated with verruca vulgaris.To reduce incidence of verruca vulgaris effectively,people should develop good living habits,give up smoking,sleep earlier,do exercise appropriately,avoid skin damage, keep a good mood,keep palms and soles dry. Part two:Clinical Efficacy of Verruca Vulgaris Treated by Acitretin Combined With Intralesional FluorouracilObjectivesTo observe the clinical efficacy and safety of acitretin combined with intralesional fluorouracil in verruca vulgaris treatment.To analyse the factors influencing the efficacy of the combination treatment.MethodsA randomized,patient-blinded,controlled clinical trial was conducted in this research.A computer-generated randomization table was used to serially allocate patients to the case or control group.The patients in the case group received acitretin combined with intralesional fluorouracil treatment. The patients in the control group received intralesional fluorouracil treatment only.All patients were followed up at the4th,8th and12th week,the clinical efficacy and side-effects were observed,and factors influencing efficacy were analyzed.SPSS13.0software was used to do the statistical analyzes,data was analyzed by x2test and Mann-Whitney U test. ResultsThree hundred and fifty patients with verruca vulgaris were enrolled,116patients in the case group and234patients in the control group.At the fourth week,there was no significant difference in clinical efficacy between the two groups(x2=0.254, P>0.05).At the eighth week,there was statistical significance for the clinical efficacy between the two groups(χ2=6.818, P<0.05).At the twelfth week,the difference became more significant (χ2=13.261, P<0.01).At the12weeks follow-up,median excellence time for the case group was7.5weeks,8.9weeks for the control group,by Mann-Whitney U test, there was statistical significance for the median excellence time between the two groups(Z=-3.455, P<0.01).Efficacy-influencing factors analysis of the case group showed that patients sex or age and the lesion’s onset or course were not related to the clinical efficacy.The distribution,number,thickness and site of warts were related to the clinical efficacy.ConclusionsThe treatment of verruca vulgaris with acitretin combined with intralesional fluorouracil was effective and safe.The combination shortens the curative time compared to a single treatment with intralesional fluorouracil.Patient’s sex or age,and the lesion’s onset or course had no relationship with the clinical efficacy,the distribution,number,thickness and site of warts were related to the clinical efficacy.The combination therapy suit specially polymorphous,multiple warts than single treatment with intralesional fluorouracil. |
