To achieve early detection early diagnosis, early interventionis to improve the key to dementia diseases，It can also significantly reduce the hazardsof AD，reduce the burden on society and the family. According to incompletestatistics.The Diagnosis of AD is through neuropsychology imaging biochemistrymolecular biology and clinical performance the neuropsychological examination scalediagnosis and treatment of AD is an important indicator to measure effectiveness. TheNew Psychometric Test Battery Used in the Bristol Memory Disorders Clinic-RevisedChinese (BMDC-NPTB-CR), Montreal cognitive amassment (MoCA) Chinese versionand mini-mental state examination (MMSE) are common scales for clinical cognitiveimpairment on screening and assessment scales.BMDC-NPTB-CR has high reliabilityand validity, compared with the MMSE and MoCA，it’s more sensitive and specific,however, because its time of application in the domestic is short so different parts hasdifferent results. This study aimed to compare the application values among BMDC-NPTB-CR、 MoCA and MMSE, also analysis the screening effect ofBMDC-NPTB-CR for early dementia，discuss itsreliability, validity sensitivity andspecificity. Discuss the best cutoff value，to assist in early detection of AD patients inthe old people.Methods:100old subjects were evaluated with BMDC-NPTB-CR、MoCA andMMSE in our neuropsychology assessment department from January,2011toDecember,2012. MMSE was considered to be the criterion validity; we compared theparallel validity of the three scales and compare the scores in different cognitive levelold subjects. All tests were carried out by trained professionals, collecting of history andto give patients physical and neurological examination. The DSM-Ⅳ and CDR weretaken to be diagnostic criteria to judge dementia, suspicious dementia or normalcognition. Statistical methods used SPSS for Windows13.0package, using analysis ofvariance to compare the means, correlation analysis using Pearson correlation analysis.Results: The Cronbach α of BMDC-NPTB-CR was0.784, and the alphacoefficients of the subscales were0.773~0.773. The correlation coefficient r betweeneach subtest scale and scale total score were0.473~0.473(P <0.001), suggesting thatthe BMDC-NPTB-CR has a good internal consistency. The total scores betweenMDC-NPTB-CR and MoCA, MDC-NPTB-CR and MMSE were highly correlated,respectively (r were0.787and0.897, P <0.05), each test scale hassignificantcorrelation, indicate that the MDC-NPTB-CR has good criterion validity. Differentcognition level subjects showed significant difference in the tests, suggesting that thesubtest and the total score of MDC-NPTB-CR can distinguish the cognition level.Conclusion: The New Psychometric Test Battery Used in the Bristol MemoryDisorders Clinic-Revised Chinese is a set of standardized and complete scale with goodreliability and validity. It can achieve comprehensive, system evaluation of multiplecognitive functions in elderly group. And it was suitable for clinical application andscientific research in our country. |
