A Clinical Analysis Of24Uterine Sarcomas

Objective: To investigate the clinical feature and prognosis of the uterinesarcomas.Methods:Retrospective analysis of clinical data of24cases of uterine sarcomapatients were treated by the First Affiliated Hospital of Dalian Medical University fromJanuary2002to January2012year. Summary its clinical features and treatment andfollow-up.Results:1、24cases of uterine sarcomas in MMMT7cases, LMS9cases, ESS6cases, UES2cases.The average age58.46±10.93years old, the main clinicalmanifestations of abnormal vaginal bleeding (41.66%) and abdominal mass (33.33%),pathological diagnosis in the preoperative diagnosis rate of81.81%(9/11).2、24patients underwent surgical treatment, basic surgical:hysterectomy+dualattachment±pelvic lymph node dissection/omentum resection, adjuvantchemotherapy7cases; chemoradiotherapy3cases; radiotherapy in1case.3、Patients were followed up for2、5years survival rates were78.2%,、61.5%, ofwhich Phase I、II survival rate was63.6%、38.4%, Phase III survival rate was13.6%、7.6%, respectively.6cases of ESS5years survival rates were100%;9casesof the LMS patients with2、5years survival rates were75%,、42.8%;6cases ofpatients with MMMT2、5years survival rates were100%、33.3%;2cases UES were50%,0%.Conclusion:1、Low incidence of uterine sarcoma, the lack of typical clinicalsymptoms, preoperative diagnosis rate is low, the main MMMT symptom is abnormalvaginal bleeding, LMS is abdominal mass,ESS often have vaginal bleeding andabdominal mass, UES have vaginal the drain;2、Ultrasound and MRI can help in the preoperative diagnosis l, LDH、CA125meaningful lesion characteristics, clinical awareness of the disease and vigilance isvery important; 3、Definitive diagnosis requires pathological examination, diagnostic curettageof MMMT meaning is larger, curettage or cervical neoplasm biopsy can help diagnoseESS, but the LMS seems to be combined with MRI to help diagnose;4、Affect the prognosis of treatment: MMMT, UES should conduct acomprehensive surgical staging, postoperative adjuvant chemotherapy, pay attention tothe LMS postoperative chemotherapy, and improve postoperative adjuvant hormonetherapy ESS.
