| Objective:To compare the treatment effect of renal angiomyolipoma(RAML) between retroperitoneal laparoscopic nephron-sparing operationand open nephron-sparing operation.Methods:The clinical data of35patients was analyzed, who sufferedfrom RAML and received partial nephrectomy.15cases were group A,who was done by retroperitoneal laparoscopic partial nephrectomy，20cases were group B, who received open partial nephrectomy.Results:Group A (laparoscopic group): the average tumor diameterwas3.3cm (2-8cm), the average volume of intraoperative bleeding was154.7ml (20-400ml), the average warm ischemia time was18min(15-30min), the average operation time was119.0min (80-165min), the averagerecovery time of intestinal function after operation was2.4days (2-3days),the average hospitalization stay after operation was13.7days (10-20days)， without conversion to open operation, blood transfusion, orpostoperative complications; Group B (open group): the average tumordiameter was4.6cm (2-10cm), the average volume of intraoperative bleeding was292.5ml (100-800ml), the average warm ischemia time was35min(30-40min), the average operation time was109.2min (40-180min), the average recovery time of intestinal function after operation was3.2days (3-4days), the average hospitalization stay after operation was13.5days (8-19days)，one patient underwent right nephrectomy because ofhemorrhage after operation, the others had no postoperative complications.laparoscopic group was less than open group in the volume ofintraoperative bleeding and the recovery time of intestinal function afteroperation(p<0.05). The patients were followed up about3-60months, notumor recured till now.Conclusion:Retroperitoneal laparoscopic nephron-sparing operation isworthy of selective application for some patients with RAML, which is akind of safe and feasible, less trauma, and quicker recovery operation. |
