ObjectiveIgA nephropathy is a common primary glomerular disease, which clinical manifestation iscomplicated, more performance for hematuria, proteinuria, with varying degrees of renalimpairment is a common cause of end-stage renal disease. Therefore, early to judge the severity ofthe IgA nephropathy is crucial. This experiment by detecting kidney KIM-1protein expressionand KIM-1in the urine protein levels, and analysis of KIM-1expression correlated with theseverity of IgA nephropathy, which reveal KIM-1protein expression in IgA nephropathy, thesignificance, for the clinical treatment of IgA nephropathy provide new effective targets.MethodsRetrospectively analyzed in May2011and January2013hospitalized after renal biopsydiagnosed IgA nephropathy patients in Ningbo urinary kidney disease hospital, according to thestandards included in the group of100patients with IgA nephropathy, the press Hass class isdivided into the I-V,20cases per level, a random of20patients selected by healthy controls inMay2011and January2013. IgA nephropathy patients after renal biopsy in kidney tissues, fixed in10%formaldehyde solution, after HE dyeing observation renal pathological morphologicalchanges, the application of immunofluorescence and immunohistochemistry method to detect KIM-1protein expression in renal tissue; Respectively to return IgA nephropathy patients kidney wearon that day morning urine and healthy controls physical examination on the same day morningurine, with ELISA method to detect the concentrations of KIM–1in the urine.Results74cases of the men and46cases of women in120patients, age was (44.1±8.7), renalhistopathological changes mainly in IgA nephropathy group diffuse glomerular mesangial cellproliferation and mesangial matrix, the individual lesions in patients with severe global hardening,as Hass hierarchical levels rise, and kidney injury severity is aggravating gradually. Withimmunofluorescence examination, IgA nephropathy group can be found and main in glomerularmesangial area, immunohistochemical visible KIM-1protein expression in IgA nephropathymainly concentrated in the proximal convoluted tubule, tan granule, at all levels of IgAnephropathy KIM-1set of integral optical density value respectively is473±95,947±143,947±143,4893±414,4893±414,12260±991, comparative differences between groups was statistically significant (P <0.05). Healthy controls KIM-1expression level in urine is extremelylow, KIM-1expression in patients with IgA nephropathy in the urine,(4.65+1.32)ng/L,(13.21+5.76)ng/L,(19.56+6.93)ng/L,(27.62+7.31)ng/L,(35.65+7.2)ng/L. Compared with the healthycontrol group, the difference was statistically significant (P <0.05), at all levels in patients withIgA nephropathy further comparison between groups, the difference was statistically significant (P<0.05).Conclusions1.as the Hass higher hierarchical levels, kidney injury severity gradually aggravate, KIM-1highly expression in renal tissues of IgA nephropathy, the expression of which are graduallyincreased on aggravating the degree of renal tissue damage;2.healthy control patients urine KIM-1expression rarely, IgA nephropathy group, KIM-1positive expression, and gradually rise, theamount of urine protein KIM-1with IgA nephropathy Hass classification is related.3. KIM-1protein expression and IgA nephropathy have close relations, the degree of damage can be used asevaluation of IgA nephropathy, a reliable indicator for judging prognosis of IgA nephropathy.
