Object: To explore the application research of dynamic contrast-enhanced MRI vessel imaging for breastlesions.Methods:1、Thirty cases were collected from january to september with unilateral breast masses whichconfirmed by biopsy or surgical pathology underwent MRI flat sweep and dynamiccontrast-enhancement,all were female, age from twenty six to sixty years old, the average age (±SD) was44.5±7.6years.Pathological types including invasive ductal carcinoma9cases,invasive carcinoma2cases,mamillary intraductal carcinoma2cases, carcinoma in situ2cases,fibroadenosis10cases,adenomafibrosum3cases,lobular hyperplasia1case,intraductal papilloma1case.2、Using GE Signa1.5T.HD MRand a proprietary phase array surface breast coil to do both and whole breasts MRI.3、With3D MIPsoftware in the AW4.3workstation, using the relatively significant strengthening peak of phase imagessubtracted the flat images,according to MIP for3D reconstruction image, the two radiologists who haveworking experience above5years to analysis and record reconstruction image through different plane androtation with high resolution MR image analysis workstation.The show of breast vessels first,the number ofblood vessels and tumor vessels in the number of speripheral lesions3cm were recorded according to themethods from Sardanellie et al. Finally,The time-signal intensity curve was drawed in Functool software,rendering the pathologic results for the standard, compared the difference between the benign andmalignant lesions.Results:1、Breast vascular displayed: the thoracic artery as the main blood within the supply arteries,benignlesions in the thoracic artery Ⅰ/Ⅱ level branch display rate were96.7%/86.7%,the lateral thoracic arteryⅠ/Ⅱ level branch display rate were80%/40%;Malignant lesions in the thoracic artery Ⅰ/Ⅱ level branchdisplay rate were100%,the lateral thoracic artery Ⅰ/Ⅱ level branch display rate were53.3%/46.7%,theⅡ level branch display rate in malignant lesions was higher than benign lesions.2、The mean (±SD)number of vessels in the benign and malignant cases was2.5±1.4and4.9±0.9(p <0.001), respectively,scores was1.7±0.6and2.9±0.3(p <0.001), respectively;The mean (±SD) number of tumor vessels whichin speripheral lesions3cm in the benign and malignant cases was1.4±1.2and4.4±0.9(p <0.001),respectively, scores was1.2±0.9and2.9±0.4(p <0.001), respectively.3、According to the morphologyanalysis,15malignant lesions were edge irregular, points leaf, heterogeneous enhancement or centralitystrengthen,and in type III curve;15benign lesions, the shape and strengthen were differ, the curvedistributed in I, II or III type.Compared with pathologic results, according to the breast vascularity in thediagnosis of breast cancer analysis of sensitivity,specificity were100%and40%, while tumor vesselsvascularity in the number of speripheral lesions3cm diagnosis of breast cancer analysis of sensitivity,specificity were100%and60%,according to time-signal intensity curve were100%and73%;Comprehensive vascular diagram analysis and time-signal intensity curve, the sensitivity andspecificity were100%and86.7%.Conclusion:The level II branch display rate in malignant was higher than benign lesions,the vascularity inmalignant cases was significantly higher than benign cases presened with the number of vessels. Especiallyin tumor vessels which in the number of speripheral lesions3cm, with significant differences betweenbenign and malignant cases. Increased vascularity had an important role in differentiating benign andmalignant breast lesions.
