The Surgical Therapy Of Involve Weight Bearing Area Acetabular Fractures And Analysis Of The Factors Of Influence Results

Objective This paper retrospectively reviewed the clinical diagnosis and surgicaltreatment of involving weight bearing area acetabular fractures, To explore the factorsthat affect the efficacy, and look forward to improve the clinical efficacy.Methods16cases of surgical treatment inpatients with involving the weight bearingarea acetabular fractures in our in our department in June2008~June2012have beenselected.Before surgery, we take three standard iconography inspections,includingJudet X-ray,anteroposterior hip obturator oblique,iliac oblique included，CT scan andthree dimensional reconstruction.By Letournel-Judet hip acetabular fracture taxonomiesclassification，16cases of involving weight bearing area acetabular fracture withdislocation were treated with open reduction and internal fixation with reconstructionplates，K-L approach was adopted in10cases,ilio inguinal approach in1cases,andanterior combined posterior approach in5case.Reviewing digital X-ray one week laterafter surgery to observe the result of fracture reduction and wound healing.Patientsfollowed up have been carried out including physical examination, X-ray and CT scanso as to proceed statistics of post-operative complications.Results16cases were followed up for6~48months.All fractures healed in3~5months.Fractures reduction quality were assessed by Matta radiographic grade: anatomic reduction9cases，satisfaction reduction6cases，dissatisfaction reduction1cases.Function of hip joint were assessed by D’Aubigne-Postel，and the followingresults:5cases were excellent，8good，2fair，1false.Traumatic arthritis was foundin3cases，heterotopic ossification in1cases.No infection，sciatic nerve injury andfemoral head necrosis were found..Conclusion According to our experience,preoperative correct grasp of the type ofweight bearing area fracture,the appropriate timing of surgery and approach, goodarticular surface reconstruction and reset, simple and effective fixed,actively preventcomplications are basis of improving the efficacy of this type of fracture.
