| 0bjectiveAt present the cancers in our country are on the rise, for example the incidence of breastcancers is increasing with the rate of three percent annually.At the same time, the incidence of stomach cancers and lung cancers also showascendant trend. Chemotherapy and drug therapy is the same concept, which is the mostimportant part of comprehensive treatment. The uaually chemotherapy drugsincluding, anthracyclines, alkylating agents, cell microtubule drugs and so on. However,chemotherapy drugs will produce some toxic effect on the bodys,so their clinicalapplication are limited. Two-dimensional speckle tracking imagingtechnology(2D-STI) is a new technology, which can quantitative display myocardialmotion speed, strain, strain rate,and the heart of the rotation angles and rotation speed,can evaluate cardiac contraction and diastolic function. This study adopts2D-STIevaluate systolic and diastolic function of left ventricle (LV) in cancer patientsreceiving chemotherapy, finding the early influence on cardiac function changes andguiding adjustment of chemotherapy regimens timely.MethodsA total of108patients receiving chemotherapy were included in this study. Every patient were studied in four stages(before chemotherapy,after the first cycle ofchemotherapy, middle cycle of chemotherapy and after the last cycle of chemotherapy)Scerum cTnT and cTnI levels were tested in every stage. The global longitudinalsystolic strain(GSL) and strain rate (GSRs) of LV as well as early-diastolic strain rate(GSRe) and late-diastolic strain rate (GSRa) were measured by2D-STI in apicallong-axis, four-chamber and two chamber views.Results1. Compared to the data before chemotherapy,only E/A had significantly difference (P<0.01).2. Comparison of the global longitudinal strain and strain rate of LV in diferentstages only the last cycle of chemotherapy had significantly difference.3. Compared to the data before chemotherapy, GSL and GSRs had decreasedsignificantly in the last stage (P<0.01).4. Compared to those before chemotherapy, the patients,GSRe and GSRa had nosignificantly difference after the first cycle chemotherapy (P>0.05), but hadsignificantly difference in the middle and the last stages (P<0.01).5. The cTnT and cTnI had significangly increased in every stage of chemotherapy andhad a gradually rising trend6. The rate of abnormal electrocardiogram(ECG)also had significantly difference (P<0.01).Conclusions1. STI can accurately evaluate the LS of patients receiving chemotherapy, whichprovides a simple,fast,repeatability and no angle method for measuring LV function forclinical. 2. The cTnT and cTnI in every stage of chemotherapy significangly increased and hada gradually rising trend,showing that the degreement of cardiac cells weredamaged also increase.3. The chemotherapy drugs first damage the LV diastolic function in patientsreceiving chemotherapy then damage systolic function.4. The rate of abnormal electrocardiogram(ECG)also had a gradually rising trend aschemotherapy. |
