Backgroud: Lumbar degenerative spondylolisthesis (LDS) is a kind of common and frequently-occurring disease clinically. It makes up a large proportion in lumbar degenerative diseases in eldly people. It is still controversial about the pathogenesis of lumbar degenerative spondylolisthesis. Most of the scholars believe thatit is interwovenness between lumbar degenerative spondylolisthesis and the intervertebral instability induced by the intervertebral disc degeneration (IVDD). Investigating the pathological role as well as the relationships of cell signal pathwaysin the process of IVDD, may further clarifies the pathogenesis of LDS and provides novel ideas and methods for clinical intervention of IVDD.Objective: To detect the expression and to analyse the clinical related factors of TGF-β1and Smad3in the intervertebral discs of people suffering lumbar dege nerative spondylolisthesis. To investigate the probability of non-surgical treatmentintervening IVDD progress.Methods: Experimental group, the First Affiliated Hospital of Anhui Medical University,April1,2011to June31,2012,42patients possessing the diagnosis of lumbar degenerative spondylolisthesis (LDS) and accepting operative treatment in this hospital. Male was16and female was26, age45~72years, mean age57.26±7.35years. According to the Meyerding Grading Systerm, there were14casesgrade â… ,19cases grade â…¡,9cases grade â…¢ and0case grade â…£, respectively.Control group consist of12cases, age38-63years, mean age51.08±7.74years.All cases were patients of traumatic lumbar spine burst fracture and without thehistory of low back pain. Immunohistochemical staining and Morphological image analytical system were used to assay the expression of TGF-β1and Smad3inthe intervertebral discs of42patients suffering lumbar degenerative spondylolisthesis. The12intervertebral discs which harvested from patients with trauma of vertebra were recruited as the control group and were assayed with the same method.Result:1. The level of TGF-β1and Smad3was both significantly higher in thestudy group (the mean grey number of TGF-β1and Smad3was62.79±18.53and32.72±13.41, respectively)than in the control group(the mean grey number was39.07±19.76and20.95±6.28, respectively, both p<0.01).2. Pearson correlation analysis showed a positive correlation between TGF-β1andSmad3expressions in the study group(r=0.709,P <0.05).3.Multiple linear regression revealed that grade classification of spondylolisthesis,course of disease and age were key factors for TGF-β1and Smad3expression. Conclusion: The TGF-β1/Smad3signaling path participate in the occurrence anddevelopment process of degenerative spondylolisthesis. There is a positive correlation between TGF-β1and Smad3expressions in the intervertebral disc of lumbardegenerative spondylolisthesis. The grade classification of spondylolisthesis, courseof disease and age were key factors for TGF-β1and Smad3expression. Modulating the expression of this path maybe a novel approach to treat the lumbar degenerative spondylolisthesis. |
