[Objective]:To evaluate the diagnostic and differential diagnosis value of MRI with in-phase and out-of-phase subtraction technology in adrenal tumors. And then compare in phase IP/opposed phase OP (evaluation indexes SII) and in-phase and out-of-phase subtraction MR imaging (evaluation indexes FF) value of the two methods in the diagnosis.[object and methods]:a retrospective analysis of40patients in adrenal tumors(male20, female20; adenoma group19, non adenoma21) with46adrenal lesions(adenoma group20, non adenoma25) underwent gradient echo MRI in-phase and out-of-phase subtraction technology in adrenal tumors. And measure the signal intensity index (SII), adrenal to spleen signal ratio (ASR), adrenal suppression index (ASI), fat fraction (FF).Using Mann-Whitney U method on non adenoma and adrenal adenoma groups, then SII, ASR, ASI, FF were statistically analyzed. Using ROC curve to calculate the optimal threshold, the sensitivity, the specificity and the area under curve (AUC) of various indexs. To compare the diagnostic accuracy of datasets reconstructed using in-phase and out-of-phase subtraction MR imaging, with corresponding opposed-phase(OP) and in-phase (IP) datasets for characterization of adrenal lesions.[results]:There were significant differences between adenomas and non-adenomas with regard to mean SII, ASR, ASI, FF difference (P<0.05). ROC curve analysis showed that the SII value of diagnosis for adrenal adenoma optimal threshold is0.297, the sensitivity was70%, specificity was96%, AUC was0.802;The ASR value of diagnosis for adrenal adenoma optimal threshold was0.847, sensitivity75%, specificity was80%, the AUC was0.782;The suppression index value of diagnosis for adrenal adenoma optimal threshold was0.164, the sensitivity was80%, specificity was88%, the AUC was0.834;The fat content value of diagnosis for adrenal adenoma optimal threshold was0.157. the sensitivity was80%, specificity was88%, the AUC was0.838.To compare in phase IP/opposed phase OP (SII) and in-phase and out-of-phase subtraction MRI (FF) accuracy of adrenal tumor was0.802;0.838.[Conclusion]:1、MRI with in-phase and out-of-phase subtraction technoloy can be used for diagnosis of adrenal urmors.2、MRI with in-phase and out-of-phase subtraction technology was beneficial for the adrenal (?)(?)nd containing lipid and non-lipid-containing tumors in the differential diag(?)s.3、The The aal method and ROCcurve analysis are two ways to show that MRI with in-phase a(?). out-of-phase subtraction technology has better diagnostic accuracy tha(?)(?)ne CS.(?)n adrenal adenomas.4、 Adrenal suppression index has higher diagnostic accuracy than signal intensity index in adenomas.5、 SII、ASI、FF can carry a certain degree of quantitative analysis on the content of adrenal tumors.
