ObjectiveTo observe the expression of PDGF-BB,PDGFR,AT1R,HIF-1α in renal biopsy tiss ue with lupus nephritis, analyze and compare clinical manifestations and laboratory test results, explore the mechanisms and significance of PDGF-BB,PDGFR,AT1R,HIF-1α i n juxtaglomerular apparatus with lupus nephritis,study the correlation with clinical mani festations, laboratory testresults and clinical score in different pathological types with L N.MethodsPatients with renal biopsy tissue samples and clinical data collected from77patients with lupus nephritis, according the International Society of Nephrology/Renal Pathology (ISN/RPS)2003classification standard for pathological type,which three cases of type Ⅰ, cases of type Ⅱ, eight cases of type Ⅲ, fifteen cases of type Ⅳ, eleven cases of type Ⅴ, thirteen cases of type Ⅲ+Ⅴ, fourteen cases of type Ⅳ+Ⅴ. By immunohistochemical methods to detect the expression of PDGF-BB,PDGFR, AT1R,HIF-1α in renal biopsy with lupus nephritis, analyze its pathological significance and mechanism,apply lupus disease activity index2000(SLEDAI2000)to determine their lupus activity and analye correlation between them with serum creatinine, complement C3,24urine protein quantitative and serum albumin indicators.Result1. LN and control kidney tissue immunohistochemical staining results showed that the expression of PDGF-BB, PDGFR, AT1R,HIF-1a in renal biopsy of LN was much higher than in the normal kidney tissue.2. Different express levels of PDGF-BB,PDGFR, AT1R, HIF-1a in different types of glomerulus and juxtaglomerular apparatus with pathology type of LN, the type of Ⅳ+Ⅴ LN was the highest.3. Between the glomerulus and the juxtaglomerular apparatus, the expression of PDGF-BB, PDGFR, AT1R, HIF-1α were highly positive correlation with the type of Ⅱ, Ⅲ, Ⅳ,Ⅴ, Ⅲ+Ⅴ, Ⅳ+Ⅴ inLN.4.1n each type of LN,the expressions of PDGF-BB,PDGFR-BB,AT1R,HIF-1a in the glomerular and juxtaglomerular apparatus were no significant correlations with activity index SLEDAI score, serum albumin, serum creatinine, complement C3,24-hour urinary protein quantitative.The SLEDAI score and serum creatinine were the highest and the C3was lowest in the type of Ⅳ LN. The24-hour urinary protein excretion was the highest in the type Ⅳ+Ⅴ.ConclusionsThe reseach founded that the expression of PDGF-BB,PDGFR,AT1R,HIF-1a in glomerular and juxtaglomerular apparatus of kidney tissue with LN were increased,the expression of type Ⅳ+Ⅴ was the highest in renal tissue with LN.And their expression, the injure extent of renal and lupus activity level were positively correlated.Suggesting that PDGF-BB,PDGFR,AT1R,HIF-1a playe an important role in the pathogenesis of development with LN, participate in the cell proliferation,inflammatory infiltration process, ischemia and hypoxia,and kidney damage into the fibrosis stage.
