| Objective:Detection the concentration levels of serum transforming growth factor-β1(TGF-β1)ã€soluble endoglin(sEng)and expression levels of TGF-β1ã€TGF-β recepter â…¡ and endoglin in placenta and myomertrium in patients with hypertensive disorder complicating pregnancy (HDCP) and normal maternal in the third trimester. Analised the relationship among these factors, and clarify the function of TGF-β1〠Endoglin in pathogenesis of the hypertensive disorder complicating pregnancy.Material and Methods:90patients were collected from April2009to-April2010in The Second Hospital of TianJin Medical University,included gestational hypertension30cases, mild preeclampsia30cases and severe preeclampsia30cases. Selected20cases of normal pregnancy at the same time.Each grounp was singleton〠primiparity, excluded the other complications and deliveried by cesarean section.ELISA was used in testing surem concentration of TGF-β1and sEng, WesternBlot was used in testing the expression level of TGF-β1ã€TGF-βRâ…¡ and Eng in placenta and myomertrium.Results:1. The systolic pressure in each group was not same, the difference has statistical significance (P<0.05); compared with the control group, the diastolic pressure in groups with HDCP were higher, and the difference has statistical significance (P<0.05), but the difference has no statistical significance between mild preeclampsia and severe preeclampsia group; cmopared with severe preeclampsia group, BMI in control and gestational hypertension group was lower and the difference has statistical significance (P<0.05); proteinuria level in severe preeclampsia group was higher than mild preeclampsia group, and the difference has statistical significance (P<0.05)2.The serum TGF-β1concentration was9.85±3.19ã€11.21±2.32ã€14.32±4.37ã€15.37±4.93(pg/ml)in controlã€gestational hypertensionã€mild preeclampsia and severe preeclampsia group before delivery,respectively;the serum sEng concentration was 2.93±0.40ã€3.43±0.63ã€5.94±0.53ã€8.23±1.04(ng/ml), respectively; and the difference has statistical significance (P<0.05)3. The concentration of TGF-β1and sEng in umbilical cord blood serum was0at the time of delivery in all patients.4. The expression level of TGFβ1in placenta was0.12±0.03ã€0.18±0.04ã€0.22±0.07ã€0.67±0.05in controlã€gestational hypertensionã€mild preeclampsia and severe preeclampsia group at the time of delivery, respectively;the expression level of Eng in placenta was0.11±0.07ã€0.15±0.05ã€0.18±0.06ã€0.43±0.04, respectively; and the expression level of TGFβRâ…¡ in placenta was0.11±0.02ã€0.26±0.05ã€0.27±0.03ã€0.88±0.09, respectively. The difference has statistical significance (P<0.05) between each group (P<0.05).5. The expression level of TGFβ1in placenta was0.13±0.02ã€0.19±0.04ã€0.24±0.06ã€0.69±0.03in controlã€gestational hypertensionã€mild preeclampsia and severe preeclampsia group at the time of delivery, respectively; the expression level of Eng in myomertrium was0.14±0.06ã€0.16±0.04ã€0.20±0.08ã€0.46±0.05respectively and the TGFβRâ…¡ expression level in myomertrium was0.15±0.03ã€0.29±0.06ã€0.31±0.04ã€0.91±0.08, respectively. The difference has statistical significance (P<0.05) between each group (P<0.05).6. In gestational hypertensionã€mild preeclampsia and severe preeclampsia group,the sEng concentration level in serum was positive correlated with the Eng expression level in placenta and was positive correlated with the myomertrium Eng expression level too;the conditions of TGF-β1was similiiar with the sEng. The placenta TGF-β1expression level was positive correlated with the Eng in placenta; the TGF-β1expression level in myomertrium was positive correlated with the myomertrium Eng, all of them had statistical significance (P<0.05)7.1n study groups, the trend of TGF-β1〠TGF-βRâ…¡ and Endoglin expression level changing in placenta and myomertrium was same, and there was position relationship among them. The r value of TGF-β1and TGF-βRâ…¡ was0.69, the r value of TGF-β1and Eng was0.75; the r value of TGF-βRâ…¡ and Eng was0.72. All of them had statistical significance (P<0.05) Conclusion:1. ELISA and Western-Blot are reliable ways in TGF-β1ã€Eng studying.2. Compared with control group, serum concentration of TGF-β1and sEng increased in HDCP groups, the severe preeclampsia group was the highest, and the gestational hypertension group was the lowest.The difference has statistical significance (P<0.05)3. Compared with control group, the expression level of TGF-β1and sEng in placenta and myomertrium increased in HDCP groups, the severe preeclampsia group was the highest, and the gestational hypertension group was the lowest.The difference has statistical significance (P<0.05)4.The serum concentration of TGF-β1and sEng was positive correlated with the expression level of TGF-β1and sEng in placenta and myomertrium, respectively.5. The level of TGF-β1and sEng was related to the severity of HDPC.They will be efficiency maskers in monitoring the occurance and development of HDCP.6. Studying and analysis the expression level of TGF-β1ã€TGFβRH and Endoglin in placenta and myomertrium of HDCP provides the theoretical basis for the role of TGF-β1and Eng in HDCP pathogenesis. |
