Objective:By the means of comparing the difference of pregnancy outcome between t-he old and the new two model of prenatal care, this study aims to make a more parti-cular knowledge of the issues of traditional prenatal examination and the identification of new prenatal examination mode which is mentioned by WHO2006in China. This study expects to block the occurrence of preeclampsia up by nutritional guidance and intervention in the process of prenatal care via the analysis of the differences of body composition and nutrient levels between patients with preeclampsia and normal pregnant women.Method:Pregnant women (gestational age<16weeks) were assessed who gets antenatal examination in the Affiliated Second Hospital of Shanxi Medical University. With the risk factors, they should be included in the high-risk prenatal care program. Without the risk factors,,the pregnant women were randomly divided into the traditional production group and4production group after obtaining the informed consent. The4check groups were divided into observation group and control group which based on whether accept the targeted nutrition guidance and intervention or not. This study was a prospective, the object were519cases in total,26patients (5%) were lost to follow-up.Result:1. There was no statistically significant difference in age, pre-pregnancy BMI, parity, education level and family income between pregnancy women of three groups.2. The traditional group of pregnant women received less nutritional guidance and related education, more prenatal examination times, inspection fees and travel expenses. Three aspects in knowledge acquisition, times of prenatal examination, inspection charges satisfaction was significantly lower than the observation group and the control group of pregnant women. Compared three groups of pregnant women in the three aspects of the degree of satisfaction, the difference was statistically significant (P<0.05); in comparison with the doctor communication satisfaction, no significant difference (P=0.135).3. The occurrence of severe preeclampsia, gestational diabetes, premature rupture of fetal membranes, premature delivery, prolonged pregnancy in the traditional group was higher than those in the observation group and the control group. Three groups in the incidence of pregnancy complications, the difference was statistically significant between severe pre-eclampsia and preterm delivery (P<0.05). Three groups of pregnant women with cesarean section were10%,18.9%,22.5%, a statistically significant difference between groups (P<0.05).4. The traditional production group had1cases of fetal death and stillbirth. Perinatal outcomes were compared between the three groups, compared the occurrence of neonatal respiratory distress syndrome, the difference was statistically significant (P<0.05), neonatal asphyxia, low birthweight, macrosomia, stillbirth, stillbirths in newborns, had no statistical difference (P>0.05).5. Compared the test results of body composition,the results of TBW, ECW, muscle weight, FFM of preeclampsia group were significantly higher than those in control group(P<0.05);comparison of FM and BF%had no significant difference (P>0.05).6. Compared two groups of pregnant women in three big nutrients, vitamins, trace elements, in addition to iodine showed no significant difference (P=0.908), the rest were statistically significant (P<0.05).Conclusions:It is not only a proper way to reduce medical costs and relieve the burden on women via implementing the new production mode of low-risk pregnant women which do not affect the outcomes of perinatal, but also could reduce the complications of mothers and infants preferably via nutrition guidance and intervention and promote the maternal and child health. In short, pregnancy nutrition guidance and intervention can effectively reduce the incidence of preeclampsia.
