Objective:To investigate the recurrent factors of large meningiomas aftermicrosurgical therapy with Sphenoid Ridge Meningioma and assist clinical treatment.Methods: A retrospective analysis of Large (4.5cm≤D<7cm)and huge (D≥7cm)meningiomas after microsurgical resection with Sphenoid Ridge Meningioma of86patients was performed upon the clinical data treatment and follow-up results collectedfrom the ones hospitalized in the neurosurgy department of the1st affiliated hospitalbetween January2006and December2011.A statistic analysis was made on the relatedfactors such as gender, age, tumor morphous, biobehaveoral, surgically resecting degreeand the pathological type.Results:24patients of these86patients were relapsed and thus the recurrence ratewas27.90%. The recurrence time ranges from17months to64months aftermicrosurgical therapy, so that the mean recurrent time is40months. Multivariateanalysis by means of Logistic regression analysis, the results of the six related factorsshowed that: gender and age have no obvious relation(P>0.05) with the recurrence,but tumor morphology, biological behavior, surgical resection and the pathologicalgrade were significantly correlated (P<0.05). Univariate Analysis by means of χ~2testand Bonferroni adjust test, the results showed that: the tumor morphology with insidetype, bigger tumor diameter and mushroomlike type patients have high recurrence rate(P<0.05); Whether there is bone change of the biological behavior have no obviousrelation(P>0.05)but involved by nerves and vessels have high recurrence rate (P< 0.05); the total resection (Simpson Ⅰ and Ⅱ) of surgical resection patients have lowrecurence rate (P<0.05); the patients with high level of pathology grade recurrence rateis higher than patients with low level, and the recurrence rate of endothelial type ishigher than other types in gradeⅠof the histopathologic subtypes.Conclusion:1.The tumor morphology, biological behavior, surgical resection andhistological grade of the sphenoid ridge large meningioma recurrence significantlycorrelated, and can be used as predictors of postoperative recurrence, and improve thesurgical resection is to avoid the most important means of tumor recurrence.2.Preoperative angiography and embolization applications can reduce surgical riskand improve operation resection.3.Postoperative residual tumor radiotherapy application can delay the recurrencetime or reduce the recurrence rate.
