Purpose: to compare the clinical features,pathological features and treatment oflung cancer between young and old patients.Methods:A retrospective review of patients with lung cancer was done. Amongall the patients,there are83cases of lung cancer patients under40years of age or just40years old with pathological confirmation had been registered in our hostipal from2000-2010,and147cases of lung cancer≥70years old with pathological confirmation atthe same time,these patients were chosen randamly.Results:there are44males and39females of young patients,with a median age of37±3.67years.In the old patients,there are96males and51females,with a median ageof74±3.96years. In accordance with the age, in all the young patients, the youngerthe patients are, the lower the incidence of lung cancer; on the other hand,in all the oldpatients, the older the patients are, the lower the incidence of lung cancer.(1) The main clinical symptom of both young and old is cough,but the numberof the young patients who has cough and chest pain are more than old patients. Youngpatients who have greater than three symptom are more than old patients. It means thatthere are no specific clinical manifestations of young patients,and easily bemisdiagnosed.(2)Adenocarcinoma is the most common type in both young and old patients,butthe number of the young patients with adenocarcinoma are more than old patients.Theold patients with squamous cell carcinoma are more than young patients.Among all thepatients with squamous cell carcinoma,old man are more than young man.(3)The young patients have more advanced-stage lung cancer than oldpatients,and bone metastasis,brain metastasis and liver metastasis are moreprevalent.There are more intrapulmonary metastasis in the old patients.(4)In the laboratory tests, Cytokerantin-19-fragment(C211)increase more commom in old patients and neuron specific enolase(NSE) increase more commom inyoung patients.(5)Different clinical stages lead to different treatment.The young patients aremore likely to be offered chemotherapy to remedy cancer alone.The young and oldpatients have the same rate of surgery and comprehensive treatment to remedy cancer.Conclusion:lung cancer in young patients occurs most frequently in patientsbetween30to40years of age. Adenocarcinoma is the most common type in youngpatients. Metastasis is frequent. In laboratory tests, nse increased more obvious. Youngpatients with imaging findings similar to elderly patients with lung cancer,with nosignificant differences. The young patients are more likely to be offered chemotherapyto remedy cancer alone.The clinical features of lung cancer showed no specical andwere frequently neglected in young patients,resulting misdiagnosis frequently.
