Objective Congenital Biliary Dilatation is inclined to occur in oriental countries.Theconservative treatment is always invalid.Resection of the choledochal cyst and bile ductjejunum Roux-en-Y anastomosis has become the standard operation for the treatment of thisdisease.However the late postoperative complications are still occur, so people want to knowhow to reduce the incidence of the late postoperative complications of the congenital biliarydilatation. The purpose of this study is to seek for the related risk factors of the latepostoperative complications of congenital biliary dilatation typeâ… patients who has undergonethe standard operation.Through this study,we hope to further improve the quality of life ofpatients of congenital biliary dilatation after operation.Methods We have collected theinformation about61cases of congenital biliary dilatation of type â… in adult patients from thegeneral hospital of Ningxia medical university and the people’s hospital of NingXia.Throughretrospective analysis and follow-up of these information,we have successfully followed up55cases with type â… .Through the collection of these55patients’ age,gender,whether havinggallstones in the cyst before the operation,whether having pancreatitis before the operation,whether having preoperative jaundice,the preoperative state of gallbladder (including thegallbladder has been removed before the operation, calculous cholecystitis and acalculouscholecystitis),the diameter of the cyst and the anastomotic diameter,we analysed theseinformation by single factor analysis at first,if age was one of the statistically significantfactors,then we analysed the statistically significant factors with the correlation analysis, atlast by Logistic regression analysis and we hope to find the relevant risk factors of the late postoperative complications of congenital biliary dilatation.Results There were13caseswith the late postoperative complications and42patients didn’t have the long termpostoperative complications.The results of the single factor analysis showed that thestatistically significant factors were age,the diameter of the cyst and whether having calculusin the cyst.The results of the correlation analysis showed that age was associated with whetherhaving calculus in the cyst (p<0.05) and the diameter of the cyst (p<0.01).At last we analysedthese statistically significant factors with the Logistic regression analysis.And the results ofthe Logistic regression analysis showed that the relevant risk factors were age,the diameter ofthe cyst and whether having gallstones in the cyst.Conclusions The adult patients withcongenital biliary dilatation type â… who is older than36years of the age and/or who has abigger cyst (the diameter of the cyst is more than10cm) and/or who has a cyst with gallstonesmay have a higher rate of the late postoperative complications.We should make a greatereffort of follow-up on these patients,then we can deal with the late postoperativecomplications as soon as possible,so we can improve the quality of life of the patients whohas undergone the operation. |
