| Objective: According to exploring the relationship between hydrogen protonmagnetic resonance spectroscopic imaging and metabolic changes in term neonateswith hypoxic-ischemic encephalopathy,we try to assess the application value ofproton magnetic resonance spectroscopic imaging in the diagnosis and neurologicalprognosis of hypoxic-ischemic encephalopathy in term neonates.Methods:37full term newborns with clinically diagnosed neonatalhypoxic-ischemic encephalopathy in our hospital in4years were used as theexperimental group,and11healthy newborns who were born in the obstetricaldepartment of our hospital in the same period were used as the control group.We tookhydrogen proton magnetic resonance spectroscopy imaging in all study objects within13days after born,measuring the area value below wave crest of metabolites of theright basal ganglia,and recorded gestational age,sex, mode of delivery,birthweight,day-old of taking the hydrogen proton magnetic resonance spectroscopyimaging,the clinic grade of neonatal hypoxic-ischemic encephalopathy in the sametime.Then we reviewed the hydrogen proton magnetic resonance spectroscopyimaging of all study objects after a month,and had the test of development quotient in6months after born,and took statistical analysis of data metioned before.Results:1. Neonatal hypoxic-ischemic encephalopathy can cause the changes ofNAA/Cr,Lac/Cr,GLx/Cr in intracranial right base ganglia,and the more serious thepatient is,the more obviously the changes happen.2.MRS values change after theneonatal period are:NAA,Cr values significantly increased,in which NAA moreobvious;ml,Cho values decreased significantly.All these changes accociated with thedelelopment and maturation of brain tissue and the repair of injury.3.The changes of NAA/Cr,NAA/Cho in the subacute phase of neonatal hypoxic-ischemicencephalopathy are highly corelative to its progosis.Conclusion: Hydrogen proton magnetic resonance spectroscopy has an importantclinical significance in the clinical diagnosis and short-term assessment of prognosisof neonatal hypoxic-ischemic encephalopathy:early check in favour of early diagnosisand clinical grading,and check in the subacute phase can help to evalute prognosis,soit is worth in-depth studying and promoting. |
