ObjectiveThe purpose of this study is going to find the influencing factor of BNP increased in sepsispatient and to explore the value of BNP in clinical diagnosis and therapeutically effect in sepsis.MethodsAll consecutive patients admitted to our intensive care unit (ICU) at the first affiliateHospital of Jinan University from February2012to March2013with a preliminary diagnosis ofsepsis. Once the patients enter our center, checkout the biological variables, such as BNP andPCT, and collected the general clinical data. Before the antibiotics therapy, obtained two sets ofblood culture specimens from different limbs. BNP and PCT were detected bychemiluminescence immunoassay and enzyme-linked fluorescent assay (ELIFA) method. Duringthe first24hours, finish the echocardiography and finish the Acute Physiology and Chronic HealthEvaluation II (APACHE II) and Sequential Organ Failure Assessment (SOFA).31patients were includedaccording to the inclusion criteria of our research. According to the result of blood culture researchobject be divided into positive blood culture and blood culture negative group.Resuts1. As a result, there were31patients included in this study, with age of (62.32±15.71)years,16males and15females,14patients died and17patients alive. Ten of thirty-onepatients in positive blood culture group, and twenty-one in blood culture negative group.2. PCT、BNP、WBC、CR、APACHEII and SOFA were significant elevated in the group ofpositive blood culture compared to the group of negative blood culture. PPCT=0.011,PBNP=0.005,PAPACHEII=0.015,PCR=0.000,PSOFA=0.013, the differences are significant.3. PCT and LVEF were selected into the multiple linear regression equation while BNP wasdependent variable. The coefficient of determination (R2) is0.806; the calibration coefficientof determination (R2adj) was0.792, the fitting equation is y (BNP)=2274.92+13.6X1(PCT) -38.46X2(LVEF). PCT was positively associated with BNP, the standardized regressioncoefficient is0.73, much larger than the LVEF standardized regression coefficient (-0.26),suggesting that the effect of PCT on BNP was greater than LVEF.Conclusions:Infection is one of the factors for plasma BNP increased in sepsis patients, It must becautious while used the BNP value for the evidence of heart failure and the fluid overload. Andcurrently whether myocardial damage can affect the BNP value of sepsis patients or not is notascertain.
