| Objective To study the imaging characteristics of multi-slice spiral CT in HepaticMetastases.Methods187patients with primary tumors were diagnosed by operation or pathology(118men,69women; age range,28~81,mean52.4years). Including gastric cancer47cases,colorectal cancer43cases, pancreatic carcinoma16cases, lung cancer18cases, breast cancer15cases, ovarian cancer36cases and gallbladder carcinoma12cases. Plain scan, arterialphase, portal venous phase and delayed phase were performed. The analysis includingposition, distribution characteristics of the metastases and the enhanced performance inevery phase. All the cases have been double-blind read by two physicians. SPSS19.0andchi-square(P<0.05) was used to make the significant analysis.Results①Focus were found in four phases,879in plain scan,1346in arterial phase,1564in portal venous phase and1107in liver equilibrium phase. Right lobe was1142(73%),left lobe422gold (27%).②53cases with metastases carcinoma migrate to hepaticsubcapsular, including34ovarian cancer and19cancer originate from other organs(P<0.05).③There are43cases from mesenteric vein,37cases show right lobe advantage,andthere are47cases from spleen vein,21cases show right lobe advantage(P<0.05).④Gallbladder carcinoma metastasis usually occur in the liver VIII, V and IV adjacent to thegallbladder bed.⑤The rate of calcification is about8.6%, including tumors from colon,gastric, ovarian and breast.⑥The rate of bull’s-eye in portal vein tumor is higher than arteryof tumor.⑦The four enhanced phases of live metastasis can be divided into two types: thegastric, breast and gallbladder showed a TDC type of gradual rise–gradual fall, colon, lung, ovarian and pancreatic showed continued going-up.⑧89cases show cystic performance,such as gastric, pancreatic, ovarian.⑨30cases have blur boundary in portal venous phase,such as lung and breast.Conclusion①Abdominal multi phase CT can improve the liver metastasis and theprimary tumor detection, portal vein phase is the best phase to show the lesion.②The rightlobe was common lobe of liver metastasis.③Hepatic subcapsular metastasis was thecharacteristic imaging of ovarian cancer metastasis to liver.④There are43cases frommesenteric vein,37cases show right lobe advantage, and there are47cases from spleen vein,20cases show right lobe advantage(P<0.05).⑤Gallbladder carcinoma metastasis occurs in theliver VIII, V and IV gallbladder bed.⑥The rate of bull’s-eye and calcification in portal veintumor is higher than artery of tumor.⑦The four enhanced phases of live metastasis can bedivided into two types: the gastric, breast and gallbladder showed a TDC type of gradual rise–gradual fall, colon, lung, ovarian and pancreatic showed continued going-up. |
