The Relative Analysis Of Carotid Color Doppler Utrasound、CT Angiography In Carotid Artery Stenosis Disease

Objective To discuss the clinical value of carotid color Doppler ultrasound（CDUS）、computerized tomography angiography(CTA) and digital subtractionangiography(DSA) using in extra cranial internal carotid artery stenosis diseaseexamination.Methods The examinations mentioned-above were performed on302patientswho diognosised possibly carotid artery stenosis disease,observe the sensitivity、specificity、 rate of accuracy and consistency in carotid color Doppler ultrasound、CTA and the twe examination united compared with DSA in carotid artery stenosisdisease.Results Using DSA as a golden standard,compare carotid color Dopplerultrasound to DSA,the sensitivity was81.72%, specificity was59.42%, rate ofaccuracy was67.16%, missed diagnosis rate was18.28%and misdiagnosis ratewas40.58%, positive rate was51.7%,negative rate was85.95%,χ~2=83.043,P<0.01,K=0.362,the ROC Area=0.671.Compare CTA toDSA,the sensitivity was74.26%, specificity was65.48%, rate of accuracy was69.41%, missed diagnosis rate was25.74%and misdiagnosis rate was34.52%,positive rate was63.52%,negative rate was75.86%,χ~2=47.581,P<0.01,K=0.391,the ROC Area=0.710.Compare carotid color Doppler ultrasound unite CTAto DSA,the sensitivity was89.62%, specificity was51.61%, rate of accuracy was69.13%, missed diagnosis rate was10.38%and misdiagnosis rate was48.39%,positive rate was61.29%，negative rate was85.33%,χ~2=44.219, P<0.01,K= 0.399.Compare carotid color Doppler ultrasound to CTA， χ~2=50.231，P<0.01,K=0.467.Conclusion It is unsatisfactory correlativity between carotid color Dopplerultrasound and CTA compared individually with DSA in the examination of carotidartery stenosis disease; It is better correlativity between carotid color Dopplerultrasound unite CTA compared with DSA,Carotid duplex ultrasound unite CTAcan be widely used in carotid artery stenosis disease early screening,but they can’treplace the DSA.
