Objective: Operation remains the most effective treatment for intrahepaticcholangiocarcinoma (ICC). This study analyzes the prognostic factors of intrahepaticcholangiocarcinoma after operation, aiming to improve the outcomes of surgicaltreatment of ICC.Methods: The clinicopathological data of102patients with ICC undergoing operationin our hospital between January2003and November2009were collected andreviewed retrospectively.65patients undergoing curative (R0) resection were furtherevaluated retrospectively.Meanwhile,analysis of correlation between the CA19-9expression level and clinicopathological factors of ICC patients was conducted.Results: The1-,3-and5-year survival rates of102patients undergoing operationwere52%,31.9%and22.8%respectively.The independent prognostic factors of thewhole group were curative resection (RR:1.688; P=0.024),CA19-9(RR:3.291;P<0.001),and AJCC stage (RR:1.448; P=0.016).Among the65patients undergoingcurative resection (R0resection),1-,3-and5-year disease free survival rates were53.8%,32.6%and26.1%;overall survival rates were72.3%,44.4%and33.4%respectively. CA19-9≥(37U/ml)(RR:3.638; P<0.001) was the only significantadverse factor for tumor recurrence.CA19-9≥3(7U/ml)(RR:5.351;P<0.001),multiple tumors (RR:2.587; P=0.016) linked to poor prognosis.The positive rate of lymph node metastasis of the positive CA19-9group (≥37U/ml) was higherthan that of the negative CA19-9group (<37U/ml)(OR:3.231;P=0.023).Conclusions: Curative resection (R0resection) is the important therapeutic measureof ICC to achieve long-term survival.CA19-9(≥37U/ml) is the adverse prognosticfactor for survival and recurrence. Elevetion of CA19-9≥(37U/ml) is highlyassociated with lymph node metastasis.
