| Objective:To assess the characteristics and outcomes of patient older than70years with laryngeal cancer. Design:The data of155patients diagnosed as laryngeal cancer in Fudan affiliated EENT Hospital from Oct2006to Oct2008was analyzed retrospectively. Results:Totally155patient were investigated, mean age75.7years old, male to female ratio24.8:1. Onset time from1-600months, median time4months. Glottic type was116cases, supraglottic type was31cases,and transglottic type was8cases. Staging:stage1and2were98cases, stage3and4were56cases. Preoperative concurrent diseases: hypertension50cases(32.3%), cardiac arrhythemia11cases(7.1%), coronary heart disease5cases(3.2%), chronic obstructive pulmonary disease6cases(3.9%), other respiratory disease4cases(2.6%), diabetes mellitus11cases(7.1%), anemia1case(0.6%). Treatment patterns:radical radiation therapy13cases(8.4%), laser14cases(11.5%), partial laryngectomy40cases(34.4%), total laryngectomy51cases(54.1%), total/partial laryngectomy with neck dissection17cases(13.9%), only palliative care or no treatment20cases(12.9%), the statistic differences were significant as compared to the whole age group(P<0.05). Palliative care and no treatment rate increased with age(P<0.05). Postoperative complications associated with preoperative concurrent diseases. One year survival rate was94.5%, while three year survival rate76.2%. Local recurrence14cases(10.9%), regional metastasis9cases(7.0%),distant metastasis6cases (4.7%), the statistic differences were not significant as compared to the whole age group(P>0.05). Conclusion:Elderly patients with laryngeal cancer have higher rates of preoperative concurrent diseases, and postoperative complications associated with preoperative concurrent diseases. Patients aged over80with laryngeal cancer have low probability of radical surgery. With appropriate treatment, elderly patients with laryngeal cancer have the same therapeutic outcomes with patients of the whole age group. |
