Objective We present a retrospective study investigating different treatments of intra-articular calcaneal fractures and find out which one is better.Methods We analysed76patients(90feet) of intra-articular calcaneal fracture in Wangjing Hospital of CACMS from2006to2011, ranking by Sanders classification of fractures. Patients are divided into two groups,with48patients(53feet) in open reduction and internal fixation group(operation group),while28patients(37feet) in Manual reduction and plaster external fixation group(conservative group). An extensive lateral approach was used in the operation, with AO calcaneal plate fixed on the fracture in the operation group. And the conservative group use plaster external fixation in plantar flexion after Manual reduction. We measured the Bohler Angle and Gissane Angle in X-ray pictures and ranked all cases with AOFAS Ankle Hindfoot Scale,to find out a better treatment of intra-articular calcaneal fractures.Result All cases were follow-up visited in an average postoperative31.5months(13-67months), including48patients in operation group, while28patients in conservative group. The operation group got a high score than the conservative in AOFAS Ankle Hindfoot Scale. The postoperative Bohler Angle and Gissane Angle present better in the operation group either, though two therapies were proved effective by statistical analysis.Conclusion For Sanders â…  fractures, we would like to choose conservative therapy. However, operation should be performed on Sanders â…¡ and â…¢ fractures.
