Objective Explore the clinical significance about the urine neutrophil gelatinase-associated lipocalin (NGAL) in early kidney damage of systemic lupus erythematosus (SLE) patients.Methods Collect urine of 124 patients with SLE (including SLE patients without nephritis and SLE nephritis patients) and 100 cases of healthy check-up, divided into four groups, A, B, C, D. A group is SLE patients without nephritis, B group is SLE patients withⅠorⅡtype SLE nephritis, C group is SLE patients withⅢorⅣtype SLE nephritis,D group is healthy check-up. Use Scattering turbidity method to measure microalbumin andαl-Microglobulin in. urine for each groups. Then use enzyme immunoassay(ELISA) to quantitatively detect the neutrophil gelatinase-associated lipocalin in urine for each groups.Result The urinary NGAL content of A、B、C、D group is 3.51±0.49、5.48±0.98、7.89±1.71、3.50±0.39ug/L; There is no significant difference in urine NGAL between A group and D group (P>0.05). There is significant difference in urine NGAL between B group and D group (P<0.01), between C group and D group (P<0.01); Among A group, B group and C group, there is significant difference in urine NGAL (P<0.05). The urinary microalbumin content of A、B、C、D group is 15.7±3.9、56.1±11.7、274.6±87.8、4.3±0.9 mg/L; The urinary microalbumin of the patients without SLE nephritis (group A) is in normal biological reference range, less than 30 mg/L; The urinary microalbumin of the patients with SLE nephritis (group B, group C) is more than normal biological reference range (30 mg/L), and there is significant difference between the two groups. The positive correlation between the urinary mAlb and NGAL is 0.84 in the SLE patients group. The urinary a 1-MG content of A、B、C、D group is 7.04±1.57、7.68±3.38、14.26±11.21、6.96±1.43 mg/L; The urinaryαl-MG abnormal increase rate of Group A, B, C, D is 0%,11.9%,40.5%,0%, the urinary NGAL abnormal increase rate is 8.9%,97.6%,97.3%,6%; so there is significant difference of SLE patients group B and group C between urinary NGAL abnormal increase rate and urinary a 1-MG abnormal increase rate. Conclusion In Different pathological types of SLE patients with nephritis,urinary NGAL increases when urinary mAlb raises; urinary NGAL can be used not only in SLE patients to judge early renal tubular interstitial damage but also to confirm that the glomerular and renal tubular damage is parallel in SLE patients with nephritis.It is related to provide theoretical basis for the relationship. The urinary NGAL for detection of SLE patients with early renal tubular interstitial damage is more sensitive than the urinaryαl-MG.
