| The reform of the primary medical institutions is a breakthrough in the new roundof medical and health system, which is related to the realization of basic medicalservices for all, also the social harmony and stability. It has become a hot and importantresearch in all social circles that how to create a rational, long-term compensationmechanism to ensure a health and smooth running of the senior medical institutions, toalleviate the problem of “medical treatment is difficult and expensive†in the masses.The reform of Changshu primary medical institutions has got high achieves in thecountry with high levels of compensation, wide compensation channel and good qualityof primary care services, affirmed by the Ministry of Health. Under the case analysismethod and foundation of public goods theory, stakeholder theory and systems theory,this thesis use interviews, site visits, collection of secondary data and comparativestudies for comprehensive and in-depth analysis of the medical institution mechanism,focusing on comparative analysis of the effect of medical institutions compensationmechanism reform in Changshu City, summing up their experience and inadequate:emphasizing on preventive health care, increasing investment in public health,strengthening the service system, broadening the compensation channel and carryingout payment reform. The absence of responsibility of the local government, the issue offinancial security, the weak awareness of cost control, the high expense of daily runningand the low proportion of other income, all make it difficult for good compensation role,the absence of other institutional reforms also makes it uneasy for compensation reform.In the end, the article concludes with policy recommendations for perfection of primarymedical institution compensation mechanism reform: strengthening the functions of thegovernment to ensure the public nature of primary medical institutions, improving the management to reduce the cost of senior medical institution run, improving thefinancing capacity to broaden the compensation channels, perfecting coordinated systemto support the primary medical institution compensation effect.The body of the thesis consists of four parts. The paper first introduces the researchbackground, significance, research ideas and methods. In the second part, it describesthe concept of medical institutions, senior medical institutions, primary medicalinstitution compensation mechanism and the principle of the compensation, analyzes theestablishment of the primary medical institution compensation mechanism by the theorytools of public goods theory, stakeholder theory and system theory. The third part takesChangshu as a case, introduces the reform measures, compares the reform effect,concludes the experience and adequate of Changshu’s primary medical institutioncompensation mechanism reform. On the basis of the case analysis, the fourth part usesystem theory to propose policy recommendations on the compensation mechanismreform perfection of our country’s primary medical institution. |
