Tuberculosis (TB) is one of the world’s most serious public health problem,China is the world’s second-largest tuberculosis country after India, and inrecent years, the incidence of TB cases are on the rise in China, with theincrease of tuberculosis, the proportion of negative bacteria substantiallyincrease, traditional laboratory methods due to its sensitivity and specificitylimitations, it brought difficulties to the early diagnosis and treatment of TB. Inrecent years, the rise of ELISPOT has opened up a new, rapid and effectivemethod for the diagnosis of tuberculosis. In this paper, we explore the clinicalvalue of the ELISPOT technology by collecting and comparing suspected TBpatients ’T.SPOT.TB test and tuberculosis antibody test results from he FirstHospital of Jilin University.Objective:To investigate the diagnostic value of ELISPOT test and tuberculosisantibody test in negative bacteria tuberculosis,to make it better application inthe clinical.Methods:The patients with suspected or pending to exclude TB during July2011toAugust2012from the First Hospital of Jilin University, Department ofRespiratory received T-SPOT. TB and tuberculosis antibody detection.According to the relevant test results and antituberculosis treatment,we colletedexperimental object data which meet the requirements. By comparing thesensitivity, Specificity,positive predictive value, negative predictive of the twodetection methods to investigate their clinical value. Result:1、The positive detection rate in the tuberculosis disease of the ELISPOTwas93.28%(111/119), significantly higher than25.53%of the tuberculosisantibody test (28/119), and the difference was statistically significant (p<0.05).2、The specificity of T.SPOT-TB test was90.64%(155/171), significantlyhigher than87.13%(149/171) of the tuberculosis antibody test, the differencewas statistically significant (p <0.05).3、The positive predictive value and negative predictive value ofT.SPOT-TB test were87.40%(111/127) and95.09%(155/163); the positivepredictive value and negative predictive value of tuberculosis antibody testswere56%(28/50) and62.08%(149/240). Compare with the positive predictivevalue and negative predictive value of T.SPOT-TB test and tuberculosisantibody test method, the differences were statistically significant (p <0.05).Conclusion:ELISPOT assay is a fast and,sensitive and specificity method for the earlydiagnosis of tuberculosis, and have a high coincidence rate in clinical,I believeit has further prospects as an important secondary diagnostic method of TB.
