| Objective:To observe the animal and clinical effect of Buyang Huanwu Decoction on the lumbar back muscle damage after lumbar posterior surgery through the animal experiments and clinical research,which can provide certain experimental and clinical evidence to the further application of Buyang Huanwu Decoction in clinical treatment of various muscle damage.Methods:Animal experiments:Forty fernale SD rats were randomly divided into control group (group A) and the experimental group (group B), each group contained20cases. All of SD rats were set into animal model of lumbar back muscle damage according to the clinical lumbar posterior operation mode. Group A were given lavage of physiological saline (5ml/kg) and Group B were given lavage of Buyang Huanwu Decoction (5ml/kg) one time each day in the postoperative first day to eighth day. A, B two groups of rats were tested the serum Creatine kinase (CK) activity in the preoperative1day and in the postoperative first day,second day, third day,fifth day before to medicine. Collecting the specimens of lumbar back muscle around the operation area to do tissue morphology observation at the postoperative ninth day. And then, observing the repairation of damage lumbar back muscle in light-microscopy and calculating the number of rebirth muscle cells. Clinical research:Forty-three patients of lumbar4/5intervertebral disc herniation between July2011to March2012were selected. According to the admission time successively, patients were randomly divided into control group (group C,n=21) and treatment group (group D,n=22).All of the patients were taken lumbar posterior surgery.Group C patients were given clinical routine treatment, Group D patients were added treatment of Buyang Huanwu Decoction on the basis of group C. All of the patients were evaluated by ODI score at3days,1months,3months after surgery, and were given MRI test to calculate multifidus muscle’s cross-sectional area(CSA) around the operation area.The data were analyzed using statistical software SPSS16.0.Results:Animal experiments:Group A compared with group B. Their CK activity value were no significant difference (P>0.05) before and after1day of surgery.They had obvious difference (P<0.05) after2days,3days,5days of surgery.Group B rats’CK activity value recovered faster during the treatment.The rebirth muscle cells’count of lumbar back muscle around the operation area have obvious difference (P<0.05) after9days of surgery. Clinical research:Group C compared with Group D.Their ODI score were no significant difference (P>0.05) after3days of surgery. They had obvious difference (P<0.05) after1month and3months of surgery. Their CSA were no significant difference (P>0.05) before surgery and had obvious difference (P<0.05) after1year of surgery.Conclution:1.Buyang Huanwu Decoction can reduce the damage degree of lumbar back muscle after lumbar posterior surgery in the early.2. Buyang Huanwu Decoction can promote skeletal muscle cell regeneration.3. Buyang Huanwu Decoction can reduce the Atrophy degree of lumbar back muscle after lumbar posterior surgery.4. Buyang Huanwu Decoction can promote the lumbar back muscle’s functional recovery after lumbar posterior surgery. |
