Objective:The aim of this study was to compare the time-course of action of rocuronium between Buyi and Han ethnic groups.Methods:52femals under general anesthesia,ASA â… -â…¡,aged25-55years and BMI20-25kg/m2were divided into2goups:group H (Han ethnic group,n=27);group B (Buyi ethnic group,n=25).Withdrawal2ml from veins of upper extremity then monitored BP,HR,ECG,SpO2,BIS.Anesthesia was induced with midazolam0.1mg/kg,fentanyl4μg/kg,TCI of propofol (target plasma concerntration2-3|ag/ml),after consciousness disappeared and BIS maintained45-55,calibrated the TOF Watch-SX,stabilized the stimulate twitch height at100%for five times.Tracheal intubation was facilitated with rocuronium0.6mg/kg.Selected IPPV model,regulated the breathing parameters to maintain the PETCO230-35mmHg. Neuromuscular block was assessed with acceleromydgraph F (TOF Watch-SX)using single stimulation (O.lHz,0.2ms) of the ulna nerve.Recorded the onset time (Ti),nonresponse time(T2),clinical duration time(T3),75%recovery time(T4) and recovery index (Ts).During the monitoring,infused Lactated Ringer’S Solution10-12ml/kg,TCI of propofol to maintain BIS45-55,no more other drugs were used,hemodynamics instability were excluded. Strated the surgery after the monitoring and used sevoflurane combined anesthesia.Result:The onset time(Ti)in group B was157.00±52.81s and125.00±35.69s in group H,it is longer in Buyi ethnic group than Han ethnic group.There were not significant differences in nonresponse time(T2),clinical duration time(T3),75%recovery time(T4) and recovery index(T5) between two ethnic groups.Conclusion:1.Compared to Han ethnic group,the onset time in Buyi ethnic group is longer.2.There’re not significant differences in nonresponse time(T2),clinical duration time(T3),75%recovery time(T4)and recovery index(T5) between the two ethnic groups.3.Ethnic groups is one of the factors which influence the time-course of action of rocunium. ORM1genotypes were detected by PCR-RFLP technology. According to the ORM1genotypes, seventy patients were divided into three groups:ORM1*F/*F1genotype (n=40),ORM1*F1/*S genotype(n=25) and (ORM1*S/*S genotype(n=5);combined ORM1*F1/*S and ORM1*S/*S genotype,the seventy patients were divided into two groups:mutational growp(ORM1*F1/*S and ORM1*S/*S genotype,n=30) and wild gvoup(ORM1*F1/*F1genotype,n=40).Result:1.There is no difference of alphal acid glycoprotein, albumin,C-reactive protein in all groups.2.The comparison of time-course of action of rocuronium:There were no significant difference in the onset and nonresponse time (T1and T2) in all groups.Compared to ORM1*F1/*F1genotype,the clinical duration time (T3),75%recovery time (T4) and recovery index (T5) were longer in both ORM1*F1/*S and ORM1*S/*S genotypes,but there’s no difference between ORM1*F1/*S and ORM1*S/*S genotypes.Compared with wild group,there’s no diffeience of onset time (T1)and nonresponse time (T2) in mutational group, the clinical duration time (T3),75%recovery time (T4)and recovery index (T5) were longer in mutational group.ConcIusionr:There’s difference in time-course of action of rocuronium in different ORM1genotypes,the mutation of ORM1genotypes induced the longer clinical duration time (T3),75%recovery time (T4)and recovery index (T5),maybe it’s because the stronger combining ability of ORM1*S alleles,then the ORM1gene polymorphism is one of genetic factors which affect the time-course of action of rocuronium. |
