Aim:To investigate the trend of serum tumor necrosis factor-a (TNF-) level changes in the patients with acute cerebral infarction especially with hemorrhagic transformation(HT),and to assess their effects、 clinical significance and the clinical feature and the risk factors of hemorrhagic transformation of cerebral infarctionMethods:198patients with cerebral infarction and45normal controls were admitted into this study from Februarv2011to June2012.There are48patients with a hemorrhagic transformation of acute and150cases of purely cerebral infarction.Radioimmunoassay continuous detection of1.4,7and10day serum levels of tumor necrosis factor-α (TNF-a) were taked in all patients and normal controls.we analysed the data comparatively and logistic regression analysis was employed to find out the risk factors of hemorrhagic transformation of cerebral infarction。Results:Serum levels of tumor necrosis factor-a (TNF-a) of cerebral infarction in patients with hemorrhagic transformation began to rise on the first day at the onset (P<0.05). to reach peak levels on the forth days (P<0.01). and then decreased gradually, but was still higher than that in normal controls group and purely cerebral infarction group on the tenth day (P<0.05). As the focal infarction size and the hematoma presented more larger, the serum TNF-α level would be more higher. In comparison with other groups, the differences were significant (P<0.05or P<0.01) Cerebral embolism, cerebral cortex infarction. big area of cerebral infarction,hyperglycemia were the independent risk factors for the hemorrhagic transformation of cerebral infarction.The thrombolytic therapy could increase the risk of hemorrhagic transformation, anticoagulation therapy was no significant correlation.Conclusion:This study suggests that the trend of serum TNF-α level changes is correlated with the occurrence and development of cerebral infarction especially with hemorrhagic transformation (HT), Serum TNF-αlevel in hemorrhagic transformation of cerebral infarction would be more higher and keep more longer.result to more injury. Cerebral embolism.cerebral cortex infarction.big area of cerebral infarction;hyperglycemia are correlated to HT,Thrombolytic therapy are correlated with the occurence of hemorrhagic transformation of cerebral infarction...
