Cluster Of Comprehensive Care And Prevention Of ICU Ventilator Associated Pneumonia In Clinical Research

ObjectiveExplore the validity of the cluster integrated care for the prevention ofVentilator Associated Pneumonia（VAP）.Methods216cases of mechanical ventilation time is greater than48h, orotrachealintubation patients from October2009to November2011at Jinzhou CentralHospital ICU as research subjects, and were randomly divided into two groups.The experimental group was109cases, a cluster of integrated care programswere used; control group was107cases, routine care programs were used. Twogroups of patients the incidence of VAP cases, pathogenic strain distribution inthe culture of the sputum of patients with VAP, two groups of patients with MVtime, mortality, economic spending were observed and compared. Theincidence of VAP, MV time, mortality and economic cost discrepancy betweentwo groups of patients were statistically analyzed using the x2test and t test.ResultsIncidence of VAP was13cases of109patients in the experimental group,the incidence rate was11.93%. In the control group of107patients, VAP was33cases, the incidence rate was30.84%,(x2=11.53P=0.001<0.01); MV time ofthe experimental group was172.92±42.13h, MV time of the control group was220.73±52.92h. There was statistically significant between the two groups (t= 2.13, P=0.021<0.05); Mortality as well as the economic cost comparison inpatients of the experimental group were lower than the control group.ConclusionsCluster integrated care programs used in the MV patients can effectivelyreduce the incidence of VAP, and shorten the duration of MV, and reducemortality in patients to reduce the patients economy to spend, is a very effectiveand comprehensive care programs for clinical widely the promotion andapplication.
