Objective Through to the infant congenital hemangioma laser intervention before and after the clinical presentation, and histopathological hemangioma morphological features,determine the level of the sICAM-1and mICAM-1,analyzes that:â‘ The clinical significance of ICAM-1in the diagnosis of congenital infant hemangioma in hyperplasia period or not.â‘¡The effect of the clinical curative infant hemangioma after the long pulse width1064nmNd: YAG laser treatment. Methods Through clinical and B super-diagnosed, collect50congenital infant hemangioma childrens in guilin medical school affiliated hospital laser cosmetic dermatology clinic from August2010to August2011.1.Use ELISA and immunohistochemistry to detect the levels of ICAM-1in serum and tissue before surgery and4weeks later,then compared them respectively.2. All children given adjustable length pulse width of1064nm Nd: YAG laser treatment,one time once a week.Follow-up the children’ clinical observational data and record before treatment and after4weeks of the treatment.Results1.50children of the hemangioma after one time laser treatment,clinical curative effect is distinct:4cases of grade â… (8%),19cases of grade â…¡(38%),20cases of grade â…¢(40%),7cases of grade â…£(14%). And in the early time,1cases of grade â… (3.85%),9cases of grade â…¡ (34.62%),12cases of grade â…¢ (46.15%),4cases of grade â…£(15.38%).In the later time,3cases of grade â… (12.5%),10cases of grade â…¡ (41.67%),8cases of grade â…¢ (33.33%),3cases of grade â…£(12.5%).2.The level of sICAM-1before the laser [(622.03±71.9μg/L] was significantly higher than the healthy controls [(197.29±38.87μg/L](P<0.01); The level of sICAM-1one month after intervention[(597.32±81.60μg/L] compared with before [(622.03±71.91μg/L],there were significant differences(P<0.01). Compare with eachother, P<0.05.3. Infant hemangioma tissue samples of50children within12months’ are typical of the pathologic hyperplasia of pathological changes of hemangioma:Flaky irregular arrangement of the endothelial cell with less intervals organizations; endothelial cell populations surrounding the formation of larger diameter sizes of microvessels, there are red blood cells in the cavity; Pericytes can be seen around the lumen.Conclusions1.ICAM-1plays an important role in neovascularization.The expression in congenital hemangioma increased when there are large number of angiogenesis;on the contrary,show a downward trend.ICAM-1may have a closely related to the hemangioma activity,can be used as a assisted examination indicators to judge the hemangioma.2. The trend of sICAM-1and mICAM-1’chances in infants with congenital hemangioma before and after laser treatment is nearly same,suggesting that the level of the sICAM-1can reflect level of the mICAM-1expression in hemangiomas.3. sICAM-1may be the index of vascular tumors in children, judge the hemangiomas is in the proliferative phase or not and determine the efficacy of secondary indexes4.Long pulse width1064nm Nd:YAG laser treatment infant hemangioma have less side effects, the method is reliable.5.The effect of laser treatment in age of6months is better than the children older than6months, suggesting that we should give clinical intervention earlier to congenital hemangioma in order to obtain better clinical results. |
