| Objective:It is a competition becomes increasingly fierce, the pace of life is to speed up in today’s societysociety. A global survey results of WHO are shown a truly healthy crowd accounts for only5%of the world’s population, the patient20%, but people under sub-health condition accounted for75%. Civil servants is a group of manage and exercise the state power, by the objective bad lifestyle and subjective all kinds of pressure, for example working pressure, complicated interpersonal relationship, long time to sit in the office, lack of physical exercise, more social activities and entertainment, etc, many people appeared a variety of symptoms of sub-health such as nervous irritability, mood is depressed, headache dizziness, memory loss, fatigue, loss of appetite, insomnia, etc. No one implement the survey about sub-health of large sample to xinjiang civil servants in china, and many current sub-health scale is bad of the reliability and validity or focusing on physiological indexes measurements, not to be widely used. SHMS V1.0scale is a research of Xujun etc. It is the quantitative measurement of sub-health from the physical, psychological and social, ect.,and it has a large sample of measurement to assess its good reliability and validity. Therefore, the purpose of the present study include:(1) It is a large sample investigation to xinjiang civil servants with SHMS V1.0, and assess its reliability and validity.(2) Find out the xinjiang civil servants sub-health status, and compare with the national average.(3) To explore the sub-health influence factors of xinjiang civil service from demographic, sociology and psychology, to provide the theory basis further for sub-healthy intervention study.Methods:1. Subjects3040civil servants of Urumqi, ChangJi autonomous prefecture, Yili area, Tacheng area and its subordinate24counties (cities) in the Xinjiang Uygur Autonomous Region.2. Operational definitionSub-health is the dropping state appears in physical, mental, or social health when there isn’t any diagnosised disease in physical or mental health. It shows declining in physical health, physical symptom, organic function, physical mobility function and vitality included. It shows bad statement in mental health, positive emotion, psychosocial symptom and cognitive function included. It shows declining in social health too, social interaction and social support included.3. InstrumentQuestionnaire of sub-health state for civil servants, which included SHMS V1.0and influence factors scale.SHMS V1.0is a quantitative measurement scale about sub-health state which is developed by Jun Xu and his teammates. Based on WHO’s definition of health, Chinese social culture and lifestyle, they developed the scale by rigorous process including Delphi expert consultation, item analysis, item selection. Large sample population test showed that SHMS V1.0had good reliability and validity.4. Data processing and statistics4.1Data processingBuild up the database by Epidata3.02and process the data by SPSS13.0.4.2Evaluation methods of SHMS V1.0In this study, we evaluate measured again reliability internal consistency reliability and split-half reliability of SHMS V1.0, and the internal consistency reliability is evaluated by Cronbach’s a coefficient. In this study we evaluate construct validity, content validity and criterion validity of SHMS V1.0, which construct validity is evaluated by correlation of items and factor analysis.4.3StatisticsDescribe sub-health state of Xingjiang civil servants by Descriptive and Frequency module. Compare converted score of SHMS V1.0among group with different demographic factors or lifestyle by two independent t-test and one way ANOVA.Find out the main factors of Xingjiang civil servants’score of SHMS V1.0and sub-health state by multiple linear regression, ordinal regression and binary logistic regression.5. Quality controlThis study is part of Chinese "863" project, evaluating and intervening system of civil servants’sub-health state, therefore we hold a scientific and reasonable plan for the investigation.Screen the questionnaires recovered, filter the ones which finished less than80%of the whole questionnaires or bad-complete-one. Build up a database by Epidata3.02, double entry and validation to minimize the deviation caused by entry.Result:1. Questionnaire recovery situationIn August2010to October finish all the survey. According to the sampling plan, in this survey,issued3000questionnaires, the recovery of2811, the recovery is93.7%. Among them the effective questionnaire was2697, accounting for95.94%of the questionnaires. After one week, issued400questionnaires, the recovery of370, the recovery is92.5%. Those tested attention on questionnaire, compliance, cooperate with higher degree.2. SHMS V1.0has good reliability and validity, and can be more comprehensive, accurately reflect the sub-health of the civil servants in xinjiang, is a reliable and effective sub-health measuring tool. Its measured again reliability is0.71(P<0.001), internal consistency reliability is0.93, Spearman-Brown split-half reliability is0.84. In structural validity evaluation, the high correlation between clauses and the dimensions of the corresponding; The eight commonality factors which produced factor analysis can explain64.37%of the Overall variation. And in the corresponding dimensions have factor loading trong, the eight factors extracted match the theoretic conception. The total score of SHMS V1.0and the total score of SF-36Spearman correlation coefficient is0.75(P<0.001).3. According to the national norm, sub-health incidence of Xingjiang civil servants reached67.33%, slightly below the national level (69.65%). In the nine dimensions of SHMS V1.0, the conversion scores of sports (P3) dimension is the highest and cognitive (M3) dimension is the lowest.4. Single factor analysis results are shown influence factors of sub-health in Xinjiang civil servants are:Gender(t=4.367,P0.000), Age(F=9.698, P=0.000), Marital status(F=10.108, P=0.000), Average daily work time(F=22.369, P=0.000), Wohnort(t=7.979,P=0.000), Living situation(F=17.062,P=0.000), Regional attribute(F=16.678,P=0.000), Unit properties(F=4.802,P=0.001), Position grade(F=9.069,P=0.000), Have a sweet tooth (t=6.7744,P=0.000), Diet salty (t=8.161,P=0.000), High fat, high protein, high quantity of heat(t=6.438,P=0.000), Engorgement (t=3.662,P=0.000), Beonadiet (t=4.568,P=0.000), Eating not on time (t=10.018,P=0.000), Smoking(F=7.691,P=0.000), Drinking(F=3.216, P=0.040), Sleep time (F=59.605,P=0.000), Computer operation time (F=6.544,P=0.000) Environment pollution(F=3.398, P=0.034), Physical exercise(F=20.865, P=0.000), Work pressure (F=56.599, P=0.000), Social friends (F=7.459,P=0.000), Social support (F=3.269, P=0.038), Free medical service (t=3.322, P=0.001), Chronic diseases (t=10.463,P=0.000), Two weeks sick (t=10.406,P=0.000), The pace of life (t=3.672, P=0.000), Family burden (t=8.575, P=0.000), Interpersonal relationship (t=7.331, P=0.000), Social resources (t=13.028, P=0.000), To the current job satisfaction (t=7.896, P=0.000), Housing Shortage (t=8.505, P=0.000), Own emotional problems (t=7.629, P=0.000), Great accident or natural disasters (t=9.894,P=0.000) 5.
