Objective: The objective of this study is by light microscope, immunohistochemistry technique and HE dyeing, to observe the pathological change and the expression of Survivin、Caspase-3in villus tissue, analyze the effects of different time of mifepristone on the villus tissue in early pregnancy and the cell apoptosis of mifepristone on early pregnancy, and provide the theory basis for rational use of medicine in clinical treatment to improve rate of abortion and reduce side reaction.Method:1. All the sixty specimens were collected from the women who desire to terminate the early pregnancy in the department of gynecology and obstetrics of Tianjin Medical college Total hospital from May1,2011to June30,2011. The30cases were pregnancy40~49days and they were randomly divided into two groups that control group A and experiment group A, The other30cases were pregnancy50~63days and they were randomly divided into two groups that control group B and experiment group B. The experiment group was treated with150microgram mifepristone before24hours. The villus tissue after uterine aspiration in all groups have been collected. Observe the pathological change of villus tissue by light microscope and.2. Detect the expression of Survivin、caspase-3in villus tissue by immunohistochemistry technique and the chorial trophoblastic cells knots by HE dyeing in different groups. Analyze the relationship between. cells knots and Survivin、caspase-3.3.Analyze the data with SPSS17.0.Results:1. HE dyeing light microscope showed: The structure of villus trophoblastic cell of control· group is clear, which the outer layer is syneytiotrophoblast and the inner layer is langhans cell. The structure is complete and the rank is compact. The experiment group showed: the cell degenerate, necrosis, even amotic, the inflammatory cell infiltrate, the villus interstitial edema, the blood siltation and the fibrin deposition.There are basically no differences between two experiment groups.2. The trophoblastic cells knots showed:In the contral group, nourish the layer of cells can see fluff leaves fit "epithelial nodules", fit to highlight the layer of cells in the epithelial villi clearance, the nodules some adhere to the surface, some mutual adhesion formation bridge kind, some with Langhans cells and some connective tissue.In the experiment groups, trophoblastic proliferation of cells forming cell nodule significantly reduced, nourish cells and cells (nourish the connection between the cell structure is flabby, intercellular increase wide.3. The expression of Survivin in villus tissue:In control group, the expression of Survivin is in the nucleus and intracytoplasm of the villus cell. The expression of Survivin in experiment groups is significant lower than control group (P<0.05), and there is no difference between two control groups. The same medicine group.4. The expression of Caspase-3in villus tissue:In experiment group, the expression of Caspase-3is in the nucleus and intracytoplasm of the villus cell. The expression of Caspase-3in control groups is significant lower than experiment group (P<0.05), and there is no difference between two control groups. The same experiment group.5. Related analysis between Survivin、caspase-3and cells knots:There was no positive correlation between Survivin and cells knots(rs=0.18),and there was no negative correlation between caspase-3and cells knots(rs=-0.17).Conclusions:1.Mifepristone can achieve the purpose of terminating pregnancy by inhibiting the the expression of Survivin, promoting the expression of caspase-3,making the villus tissue degenerate and necrosis, then inducing abortion.2. The normal villus tissue of early pregnancy have complete structure. It will change after oral mifepristone. The villus cell turned to dropsy,degenerate and necrosis.The chorial trophoblastic cells knots will reduce Significantly.3. Survivin express in the normal villus tissue of early pregnancy. The expression has already been inhibited after oral mifepristone. The antiapoptotic mechanisms of villus tissue has been destroyed and the pregnancy is hard to maintain.4.Caspase-3express high in the experiment group,and promote apoptosis.5. There was no positive correlation between Survivin and cells knots,and there was no negative correlation between caspase-3and cells knots.6.The expression Survivin、caspase-3of villus tissue in early pregnancy make no difference obviously. It can be considered in theory that the150microgram mifepristone has the same effect with medical abortion on the women between trimester of pregnancy40~49days and50~63days.
