Objective:To systematically evaluate the randomized controlled trials(RCT) of clinical effects of Buyang Huangwu Decoction (BYHWD) and its active constituent on diabetic peripheral neuropathy. This study focuses on the RCT literature about BYHWT traiting DPN in the past20years. Evaluating the quality of the literature about DPN clinical efficacy observation, with effect analysis, in order to understand the quality of literature, as well as safety and efficacy of BYHWD for alleviating DPN, which provide evidence-based clinical medical basis for Chinese traditional medicine.Method:In this study, the Cochrane network for standard manual work, literature retrieval with manual method by using the computer. We searched the following electronic databases:Chongqing VIP Database, Wan fang Database and so on. Computer retrieval in Chinese database is given priority to, taking keywords combined with vice keywords (extended all the tree) retrieval method. When necessary manual retrieval relevant professional material and network information. Strict screening, select the information that can be incorporated, systematic reviews to the literature according to Jadad scale after improving. Detection of heterogeneity was taken by the Rev.Man 5.1software provide by the Cochrane network, funnel plots to analysis published bias.Results:1.Total of176reports was found, while only8RCTs met the inclusion criteria. The methodological quality of trails was assessed by the Jadad-scald, all are the low-quality literature.2.As BYHWD (treatment group) was compared with control group, the meta-analysis indicated that Risk Ratio (Risk farther, RR) for the total effective rate was1.42and the95%(confide-nce interal, C1) is from1.28to1.58, funnel flots suggests the presence of publicated bias; the weighted mean difference (WMD) in symptom scores between the two groups is1.07, and95%CI is from0.81to1.33.WMD in the leftsural movement nerve conduction velocity between the two groups is3.79, and95%CI is from2.62to4.95,WMD in sensory nerve conduction velocity between the two groups was3.97, and95%CI was from2.93to5.01, RR between the two groups in ankle-reflex to improve was1.30, and95%CI is from0.96to1.75,WMD in plasma viscosity between the two groups is-0.14, and95%CI is from-0.23~-0.05. Two groups of comparisons, BYHWT for alleviating DPN effectively, can improve the clinical symptoms, nerve conduction speed, blood viscosity etc, and is better than the vitamin B treatment, but the efficacy of ankle-reflex is no obvious difference.Conclusion:1.There is exact effect of BYHWD and its active constituent to DPN on cure rate, and is better than the vitamin B, which mainly displaying in improved clinical symptoms, nerve conduction velocity and plasma viscosity. The experimental processes of ankle-jerk are no obvious adverse reaction.2. Into the literature evaluation, there is insufficient sample size, inconsistent diagnostic and therapeutic effect judgment standard, poorly randomized operation, insufficiency blinded application, the statistical analysis method description is simple, without mentioning lost to follow-up and exit case.3.For low-quality literature, the application of the above conclusions should be cautious, While further more high-quality trials are necessary, BYHWD in the treatment of DPN should be evaluated by more rigorously designed.
