| Objective:With the cross-sectional survey method, to collect the baseline of the hypertensive population and Traditional Chineses Medicine syndromes of the population in Pingyuan county, which is in order to make an investigation of TCM syndromes and related factors of hypertension in a large sample survey, then to provide the basis for TCM syndrome and treatment based on syndrome differention in clinical of hypertension, All this is to achieve the goal of making the high blood pressure control keys "forward" and control strategy "down", to provide the basis for the development of control strategies of community hypertension.Methods:With a large sample cross-sectional survey method to composite a sample of1835with hypertension at the same time without serious comorbidities patients in the authorities career units of Pingyuan county in Shandong province from july2008to mid-January2012,this project is depended on the two project named" Medical prevention and treatment of hypertension population works in Pingyuan county" and "teachers vascular health support plan in Pingyuan county", the full programme is according to strict standard operating procedures, through a standard questionnaire to collect gender, age, body mass index (BMI), waist-hip ratio (WHR), the number of smoking and drinking cases, family history of hypertension and clinical variable data and syndrome classification of quantitative information. According to Chinese medicine clinical research guiding principles to establish the single syndrome of such as Liver fire hyperactivity, abundance of phlegm,yin deficiency and yang excess, Yin and Yang deficiency and the relevant resultant syndrome as a standard card, to compare the difference of clinical variable data between different syndromes; According to Chinese medicine clinical research guiding principles to establish the kidney deficiency syndrome and the non-kidney deficiency syndrome and compare the difference of clinical variable data between them. All the results are imported into ACCESS database to establish a data query, statistical analysis of data is Imported into SPSS17.0version of the statistical software, and by logistic regression analysis to see the influence factors on the differences between the various syndromes.Results:(1)The survey found that the prevalence of hypertension was47.6%in the region,50.5%for men and43.3%for women, the male is significant more than female, The awareness rate was81.9%,the treatment rate was58.5%and the reaching standard rate was21%,the "rate" women were significantly higher than men.(2)With age increasing, male and female systolic blood pressure were tested the tendency to increase, while diastolic blood pressure increased first and then decreased. Women’s systolic blood pressure is changed more significant while the men’s diastolic blood pressure is changed more significant.(3)By logistic regression analysis found that sex,age,BMI,WHR,level of education,smoke,drink,family history of hypertension, medical history of stroke,coronary disease,diabetes and hyperlipemia were factors that influence hypertension, and BMI had a relatively large impact.(4)In the samples of a single standard syndromes, the rate of Liver fire hyperactivity, abundance of phlegm,yin deficiency and yang excess, Yin and Yang deficiency is respectively15.76%,5.35%,39.85%,39.04%(P<0.05). Yin deficiency and yang excess and Yin and Yang deficiency together accounted for78.89%of all people as the main composition. Liver fire hyperactivity syndrome is significantly reduced as the age increase, yin deficiency and yang excess syndrome showing decreased after increasing trend. Yin and Yang deficiency syndrome with increasing age was significantly increased. Through the kidney deficiency syndrome and the non-kidney deficiency syndrome, fonud that the kidney deficiency syndrome is significant more than the non-kidney deficiency syndrome (P<0.05),and in kidney deficiency syndrome women is more than men (P<0.05).(5)By logistic regression analysis found that sex,age,WHR,family history of hypertension,medical history of stroke and hyperlipemia were factors that influence Yin and Yang deficiency syndrome of hypertension,and medical history of stroke had a relatively large impact; sex, BMI, medical history of diabetes were factors that influence yin deficiency and yang excess syndrome of hypertension,and sex had a relatively large impact;age,HR, medical history of stroke and coronary disease were factors that influence kidney deficiency syndrome of hypertension,and age had a relatively large impact.(6)In the crowd that complicated with hyperlipidemic, the yin deficiency and yang exces syndrome and Yin and Yang deficiency syndrome was significantly less than Liver fire hyperactivity(p<0.05), Yin and Yang deficiency syndrome was significantly less than abundance of phlegm(p<0.05). Patients with non-kidney deficiency syndrome was significantly more than that with kidney deficiency (p<0.05).Conclusion:(1)By logistic regression analysis found that age,HR,medical history of stroke,coronary disease were factors that influence kidney deficiency syndrome of hypertension,and age had a relatively large impact., and age had a relatively large impact.(2)Hypertension in the middle-aged sample showed the main syndrome was Liver fire hyperactivity and yin deficiency and yang exces, and mainly double-hypertension.The elderly sample showed the main syndrome was Yin and Yang deficiency (kidney deficiency), and mainly isolated systolic hypertension. Accompanied by high blood pressure and duration of course of disease, the syndrome presented by from liver to kidney, from a standard sub-type syndrome to deficiency syndrome—the excess of liver and deficiency of kidney.(3)There was a high incidence of hypertension among the staff in Pingyuan county of organs and institutions, the rate of awareness and control rate was than the national rate,this suggesting that the prevention of hypertension has made some progress in Pingyuan county, especially a lot of work in earlier stage of Chinese Demonstration Community had been effective. A low rate that reaching the standard, suggesting that the treatment of hypertension was still not optimic, the prevention and treatment work of hypertension with Traditional Chinese Medicine still has a long way to go. |
