The Intervention Studies Of Alprostadil Caused Phlebitis

ObjectiveAlProstadil could dilate blood vessels, inhibit Platelet aggregation, reduce blood viscosity and erythrocyte aggregation, imProve microcirculation, Prevent atheroma and imProve neurological damage. And alProstadil could relieve the ischemia rePerfusion injury of the ischemia myocardium, imProve the Prognosis of revascularization in Patients with acute myocardial infarction. Therefore alProstadil was widely used in the treatment of coronary heart disease, angina, heart failure and the Patients received coronary interventional theraPy.But PGE1caused the occurrence of Phlebitis during its injection, as it is one kind of inflammatory substances. It has rePorted in the Patients who received PGE1theraPy, the incidence of Phlebitis was uP to90%. Phlebitis discomfort the Patients, and disturbed the medical treatment and nursing care.Studies have rePorted the infusion sPeed of alProstadil influence the Phlebitis occurrence. There also have been various methods to treat Phlebitis, as hot comPress, Hirudoid cream and TDP lamP were all widely used. But if all the methods are suitable for alProstadil induced Phlebitis is still not clear. The research was designed to reduce the alProstadil induced Phlebitis rate, analysis the methods of Prevention and treatment of alProstadil Phlebitis, in order to facilitate the successful aPPlication of alProstadil in clinic and enhance the treatment effect.MethodsThe in-Patients of Cardiology from March2011to January2012were selected into the study.156Patients were randomly divided into control and experimental grouPs. The intravenous injection’s time was2minutes in experimental grouP, and10minutes in the control grouP, respectively. The incidence of Phlebitis and the severities of Pain were comPared.Another68Patients received hot comPress before alProstadil was injected,62Patients did not received hot comPress. Among the Patients with Phlebitis,24received TDP and Hirudoid cream theraPy,24Patients only received Hirudoid cream theraPy. Phlebitis classification was used INS classification standard. Pain severity criteria:sharP Pain was unbearable Pain, usually accomPanied by Phlebitis; comParable Pain was the Pain can be endured by the Patients, a few Patients had Phlebitis; hyPodynia was only slight Pain, without Phlebitis; Painless was no feeling of Pain.Results1. Two kinds of alProstadil injection sPeed influenced thef Phlebitis and vein PainThe incidence of Phlebitis and vein Pain were significantly low in the observation grouP (P<0.05). While the severity of Phlebitis and vein Pain between the grouPs had no statistical significance (P>0.05).2. Hot comPress influenced Phlebitis and vein Pain.The incidence of Phlebitis and vein Pain were significantly low in the hot comPress grouP (P<0.05). While the severity of Phlebitis and vein Pain between the grouPs had no statistical significance (P>0.05).3. TDP and Hirudoid in the treatment of PhlebitisIn observation grouP, the Pain relief time is35.6±14.3minutes, inflammation subsided time is175.2±74.3minutes; In control grouP, the Pain relief time is71.5±25.5minutes, the inflammation subsided time is603.4±35.2minutes. Both the Pain relief time and the inflammation subsided time were all shorter in the observation grouP than the control grouP (P<0.05).conclusion1. Fast sPeed of intravenous bolus injection could significantly decrease the incidence of alProstadil induced Phlebitis and vein Pain, while the severity of Phlebitis and vein Pain was not influenced.2. Hot comPress could significantly decrease the incidence of alProstadil induced Phlebitis and vein Pain, while the severity of Phlebitis and vein Pain was not influenced.3. TDP in the combination of Hirudoid cream had better curative effect than in the treatment of alProstadil induced Phlebitis.
