Objective: to identify the correlation between N-terminal pro-brainnatriuretic peptide (NT-proBNP) with diastolic function in asymptomaticpatients at risk of developing heart failure, and the role of NT-proBNP inidentifying diastolic dysfunction.Methods: eighty patients (age45–70years) were selected based onhistory of hypertension, diabetes mellitus, and coronary artery disease.Patients with at least one risk factor for HF were selected in this study.Patients were enrolled in this study from January to July2011in FirstAffiliated Hospital of Chongqing Medical University. Left ventricular (LV)diastolic function was measured by conventional Doppler and TissueDoppler imaging (TDI) echocardiography of mitral annulus. PlasmaNT-proBNP concentration was measured by immunoassay.Results: There were36men and44women in this study. The mean age ofpatients was58.8years old. Hypertension was present in78/80patients(97.5%); diabetes in32/80patients (40%); and coronary artery disease in 25/80(31.3%). In transmitral pulsed-wave Doppler velocity measurement,there was no significant difference of NT-proBNP between normal andimpaired relaxation (mild diastolic dysfunction). Patients withpseudonormal filling (262.2±55.63pg/ml) and restrictive like pattern(781.5±152pg/ml) had significantly higher NT-proBNP level (p<0.0005).In TDI measurement, patients were grouped into3groups based on E/E’ratio <8(group1), E/E’ ratio8-15(group2), E/E’ ratio>15(group3). TheNT-proBNP level in group1was47.54±34.08pg/ml, group2was94.98±78.88pg/ml, and group3was781.5±152pg/ml. There was goodcorrelation between NT-proBNP and E/E’lateral (r=063; p<0.0005).Conclusion: NT-proBNP level increases as left ventricular diastolicfunction decreases, and there was good correlation between NT-proBNPwith E/E’ ratio. Patient with moderate-severe diastolic dysfunction hadhigher NT-proBNP level. NT-proBNP cannot be used for identification ofmild diastolic dysfunction in patient at risk of developing heart failure.
