Objective:To investigate the AIDS-related behavioral characteristics, infection rate, genetic subtype and drug resistance of HIV-1among the AIDS high-risk population.Methods:During April to June2011, three cross-sectional surveys were conducted in drug users, men who have sex with men (MSM), and STD clinic male attenders respectively in Tianjin. The main contents of investigation included the general demographic information (age, gender, marriage, residence, nationality, education, etc.), the behavioral information (sexual behavior, injection drug use, etc.), and the AIDS prevention and control information (the awareness of AIDS prevention and control knowledge, the situation of acceptance testing and behavior intervention services, etc.).3-5ml venous blood sample was obtained and HIV antibody test and imprinted protein (Western Blot, WB) test were done for screening and confirmation of HIV-1infection. HIV-1gag gene and pol gene were amplified by RT-PCR and sequence determination were done in the subjects with HIV-1positive for subtype and drug resistance associated multations analysis of HIV-1.Results:1The characteristics, AIDS-related knowledge, HIV-1infection rate in high-risk population1.1The result of investigation among drug users:The average age of400drug users was (33.7±8.1) years and most of them were young men.77.8%of drug users had education of junior high school and below.56.8%had injected drug.18.1%shared drug syringes.18.1%used condom in the recent sexual behavior.12.5%used condom every time when they had sex with regular sexual partners.41.3%used condom every time when they had commercial sexual behavior. The awareness rate of AIDS knowledge was69.3%.34.8%received condom publicity release or HIV counseling in recent years,15%received the community drug maintenance therapy or clean needles exchange,30.8%did AIDS detection. HIV infection rate was0.8%.1.2The result of investigation among MSM:The average age of403MSM was (28.9±8.8) years.89.1%have lived in Tianjin for more than2years. Most of MSM had higher education.10.6%of MSM used condom every time when they had sex with man.3.5%had commercial sexual behavior with man in the recent six months, and11.4%had sex with female in the recent six months. The awareness rate of AIDS knowledge rate was82.1%.45.4%had AIDS detection in the recent years. HIV infection rate was3.5%.1.3The result of investigation among STD clinic male attenders:The average age of800STD clinic male attenders was (34.9±11.6) years.53.3%were married.47.4%had sex with commercial sex worders(CSWS) in recent three months.29.1%had sex with the temporary partners in recent three months.9.4%had anal sex. The awareness rate of AIDS knowledge rate was98.6%.16.1%had AIDS detection.20.9%had previously been diagnosed with venereal disease. HIV infection rate was2.6%. The subjects, who had commercial sex, temporary sex or anal sex, had much higher infection rate than those did not (P<0.05).2Distribution of the virus subtype of HIV-1:Gag gene fragments of HIV-1were successfully amplified in all of38subjects with HIV-1positive, among whom71.1%(27/38) were CRF01_A E subtype,18.4%(7/38) were B subtype, and10.5%(4/38) were CRF07_BC subtype. All of three intravenous drug users infected with HIV-1were CRF07_BC subtype.78.6%MSM infected with HIV-1were CRF01A_E subtype and21.4%were B subtype.76.2%STD clinic male attenders infected with HIV-1were CRF01AE subtype,19.0%B subtype, and4.8%CRF07BC subtype.3Drug resistance analysis of HIV-1:Pol gene fragments of HIV-1were successfully amplified in30subjectes with HIV-1positive, of whom23.3%had drug resistance-associated mutations.Conclusion:The risk behaviors of HIV infection were prevalent among thse AIDS high-risk population. The separation of cognition and behavior existed in MSM and STD clinic male attenders. Three study population had HIV infection with different degree. Currently, there are at least3kinds of HIV-1gene subtypes in Tianjin, and the different distribution of HIV-1genetic subtype among the different AIDS high-risk population, and25%had drug resistance genes.
