| Objective:To compare the difference of HOAs from the anterior surface of age ralated cataract patients’cornea that measured by Pentacam system and Tracey wavefront aberrometer.The preliminary study of the factor that affects the cornea higher order aberration.To compare the visual quality that sfter iTrace Aberrometer and Pentacam system respectively guided personalize aspheric intraocular lens (IOL) implantation. Discussing the feasibility of useing the two instruments respectively to guide IOL implantation that the whole eye’s spherical aberration for0.1um.Methods:This research is prospective clinical research。134eyes of97person were enrolled in this study. According to the age and gender, they were divided into four groups.The root—mean—square(RMS) of spherical aberration(SA), coma, trefoil were measured with Pentacam and Tracey. Separately, we record all patients’ cornea front surface aberration under3mm pupil diameter and the5mm pupil diameter.One day before Phacoemulsification and aspheric IOL implantation surgery, We use Pentacam system and iTrace aberrometer separately to measure the corneal spherical aberration (Z40) under6mm pupil diameter of patients with age-related cataract. These patients total55patients (58eyes) is split into iTrace group (31eyes), Pentacam group (27eyes). According to measurement results, we have implanted TecnisIOL, AcrySof IQ IOL, ADAPT-AO IOL for these patients respectively,so that the patient’s predictive value of the whole eye’s spherical aberration in the range0.05-0.15after surgery. After3months, we observed the patient’s visual acuity, best corrected visual acuity (BCVA), pupil modulation trarster functions (MTF) under6.0mm diameter, spherical aberration of the whole and inside eye.Results:The result of cornea front surface aberration measured with Pentacam and Tracey:(1)Under3mm pupil diameter, the difference of middle-aged females’coma was not statistically significant (P>0.05); trefoil of elderly women group showed statistical differences between Pentacam and Tracey(P<0.05).(2)Under5mm pupil diameter, the difference of the trefoil that older females and middle-aged people’s was not statistically significant (P>0.05), the difference of middle-aged females’coma measured by pentacam and Tracey was not statistically significant (P>0.05).(3)In any pupil diameter, spherical aberration of middle-aged and elderly people, coma of elderly people and middle-aged men, trefoil of older men showed statistical differences between Pentacam and Tracey(P<0.05). The result of iTrace instrument:.(1)The corneal aberration of every group under5mm pupil diameter is higher than the corneal aberration of every group under3mm pupil diameter and difference was statistically significant (P<0.05).(2) Each group of men crowd’s aberration and each group of women crowd’s aberration’s compare have non-statistics significance.(P>0.05).(3) Under3mm pupil diameter, the difference of trefoil between older people and middle-aged crowd was not statistically significant (P>0.05).Under5mm pupil diameter, trefoil of older people is higher than middle-aged crowd and difference was statistically significant (P<0.05); In any pupil diameter, corneal coma of older people is higher than middle-aged crowd and difference was statistically significant (P<0.05)and the difference that between old age crowd’s spherical aberration and middle-aged crowd’s spherical have non-statistics significance (P>0.05)Comparison of postoperative patients, results:(1) After surgery, to campare the uncorrected visual acuity and BCVA of2groups’patients, the difference were not statistically significant (uncorrected visual acuity t=0.832, P>0.05; BCVA t=0.373, P>0.05)(2) Comparing all preoperative patients’ predictable whole eye’s spherical aberration under6mm pupil diameter and0.1, the differences were not statistically significant (iTrace group:t=-1.697, P=0.101; Pentacam group:t=0.640, P=0.528); After surgery, two groups were compared the whole eyes’actual spherical aberration under the6.0mm pupil diameter and0.1, iTrace group difference was not significantly (t=0.578, P=0.568), Pentacam group difference was statistically significant (t=2.334, P=0.028).(3) The two groups were were compared the whole eyes’predicted values of preoperative spherical aberration and the actual value of the whole eyes’spherical aberration After surgery, the difference was not significantly (iTrace group:t=-1.554, P=0.131; Pentacam group:t=-2.035. P=0.052) Two groups were compared actual spherical aberration of inside eye after surgery and IOL’s spherical aberration, differences were not statistically significant (iTrace group: t=-1.553, P=0.131; Pentacam group t=0.731, P=0.471). Difference between the two groups’actual value of the whole eye’s spherical aberration was no significant (t=1.525, P=0.133). After surgery, under6mm pupil diameter, in5,10,15,20,25,30cycle per degree (cpd) spatial frequencies, the difference of the value that the two groups’higher-order aberrations’MTF and spherical aberration’s MTF were not statistically significant (P>0.05).Conclusion:Some differences are found in Age-related cataract patients’corneal HOAs between Pcntacam and Tracey methods. The pupil diameter influence corneal aberration. The pupil diameter is bigger; the corneal higher order aberration is bigger. The gender is not the influencing factor of the middle and old aged cataract patients’ corneal higher order aberration. The age is the influencing factor of corneal coma and corneal trefoil aberration.The two groups that two instruments guided aspheric IOL implantation respectively can achieve satisfactory target of spherical aberration, iTrace group’s predictive value is more close to the actual value. The aspheric IOL implantation surgery that both the two instruments guided can get a good postoperative visual quality. |
