Background: Studies have shown that the plasma or serum cell-freeEpstein-Barr virus (EBV) DNA level might be a tumor marker for the diagnosisof recurrent or metastatic nasopharyngeal carcinoma (NPC).Objective: To evaluate the diagnostic value of the detection of EBV DNAin plasma or serum for recurrent or metastatic nasopharyngeal carcinoma.Methods: Articles related to the diagnosis of recurrent or metastaticnasopharyngeal carcinoma by the detection of EBV DNA in plasma or serumwere retrieved from MEDLINE, EMBASE, the CBMdisc databases and someother databases.Related journals were also searched manually.Sensitivity,specificity, summary receiver operator curves, and likelihood ratios were pooledto assess the diagnostic value of individual diagnostic tests by two researchers.Results:22studies were included in the meta-analysis.1709subjects wereincluded, among whom there were571subjects were recurrent or metastaticpatients and1138were relapsed patients.The pooled sensitivity and specificitywere0.85%(95%confidence interval [CI],82.0%-88.0%) and90.0%(95%CI,88.0%-91.0%). PLR(positive likelihood ratio) was7.11(95%CI5.88-8.59),NLR (negative likelihood ratio) was0.15(95%CI0.10-0.24), DOR (diagnost ic odds ratio) was52.00(95%CI33.36-81.06). The area under SROC curve was0.9451. Q index was0.8840. The subgroup analysis demonstrated that thepooled sensitivity and specificity of FQ-PCR were0.88(95%CI0.83-0.91) and0.85(95%CI0.82-0.88). The pooled sensitivity and specificity of RQ-PCRwere0.86(95%CI0.81-0.90) and0.94(95%CI0.91-0.96).Conclusion: The detection of EBV DNA for the diagnosis of recurrent ormetastatic nasopharyngeal carcinoma has good sensitivity and specificity andmight be helpful for the monitoring of recurrent or metastatic nasopharyngealcarcinoma.
